Myerstown Nursing and Rehab sits at 7 W Park Ave, near the corner of W Park Ave and New Aly, and is part of StoneRidge Retirement Living Communities, which is a Christian, not-for-profit group in Pennsylvania, and this place offers a range of care options, such as independent living, personal care, short-term rehab, skilled nursing, and memory care, so folks can stay as their needs change. The facility provides licensed nurses, personal care services, and therapies like physical, occupational, and speech, so you see people getting help with bathing, dressing, taking medications, meals, and housekeeping if they need it, and there's support every day from morning to evening, seven days a week. There are private and semi-private rooms with in-room TVs, and the building has things like a cafe, chapel, library, woodworking shop, fitness and wellness programs, and a swimming pool, along with a bank and a little convenience store for daily needs. Residents take part in different activities, games, and outings, and there's a chef preparing meals, which is a nice touch, and the food gets served in a shared dining area. Some people live in apartments or cottages, so there's more than one option for where to set up a room or a small home. The staff and the whole place try to follow Christian values, aiming for a community where people feel welcome and comfortable, and the focus is on helping folks stay as independent as they can while being there to help when needed, and everyone can join in activities or find a quiet space in the library or chapel. The facility tries to stay connected to the greater Myerstown area with access to local health care, and the community runs as a Continuing Care Retirement Community (CCRC), so people can move between different levels of care without leaving the campus, which means you don't have to uproot when things change health-wise, and that's a relief for a lot of families, and the range of amenities and activities does help keep daily life engaging and a bit easier to manage as people age.

Good care but management issues
I had a mixed experience. The therapists, many nurses and aides, activities and food were genuinely good and some staff went above and beyond, but management and staffing were inconsistent. I saw long call-button waits (~40 minutes), missed showers and laundry, diabetic/mealtime lapses, rude behavior from some CNAs/head nurse (even staff grabbing phones trying to delete photos), and unsafe practices - two-person assists ignored, a fall with injury and what felt like a premature/forced discharge. I appreciate the caring people there, but the safety and management issues make me hesitant to recommend it without improvement.
