Autumn Leaves of Stone Oak is a family-owned assisted living community designed for seniors, especially those living with Alzheimer's or other types of dementia, and the whole place has a warm, supportive feeling with staff trained in dementia care, and if residents have special needs in memory care, they can stay in a secure, purpose-built area with wander alert technology to keep them safe, which becomes pretty important for folks who tend to wander or have big changes in behavior, and the staff stays awake around the clock so someone's always ready to help with meals, bathing, dressing, medication, or transfers-whether it's a simple standby or if they need a lift or extra help. The building's got fifty beds, a license from the state (ALZ, #105829), and the place is made for comfort with both studio and semi-private apartments, pet-friendly rules so some folks can keep their dogs or cats, rooms that come already furnished but also have kitchenettes and bathrooms that work well for wheelchairs or walkers, and all the rooms have cable, WiFi, and phone lines set up so staying in touch with family is simple, and there's a call system in every room for safety. There's meal service in a dining room with restaurant-style options-including special diets like vegetarian, low sugar, low salt, or vegan, if a doctor recommends it-and the kitchen staff can handle allergies and other special food needs, plus guests are welcome at meals when residents want to eat with their families. Autumn Leaves of Stone Oak offers a range of care, from independent living with some resort-style amenities and social activities, all the way to skilled nursing for folks who need medical treatments and round-the-clock care, and maybe that's why they can offer both hospice and respite care for short stays when a caregiver needs a break. Nurses and other health professionals visit regularly, including therapists and dentists, and onsite rehab is available too, so lots of medical help comes right to the building. Residents often join clubs and activities, with a full-time activity director to help organize things like a cooking club, gardening group, musical group, book discussions, and story time, or game nights, and there's everything from religious services and lectures to offsite outings, so no two days are ever the same, and they have a recreation room and common areas with TVs and books. There's a pretty outdoor garden, patios, a dog park for pet owners, and a beauty salon and barber shop onsite, plus a pharmacy and laundry (including dry cleaning), and staff takes care of housekeeping, linens, and even move-in coordination. Security's tight, with locked grounds, and the building's laid out so folks can age in place and stay as their needs change, which is important for those with dementia or Alzheimer's. Residents get help with daily activities as needed and have mental wellness and medication programs, behavior care, and memory-enhancing activities based on what each person likes and has done throughout life, because staff develops care plans by looking closely at both a resident's health and background, and families get frequent updates and help with financial and VA benefits advice. Some apartments are semi-private, and all areas are accessible; guests can park on site. There's an entry fee and rates are all-inclusive, and the community only accepts seniors age 55 or older. Everything about Autumn Leaves of Stone Oak is set up to support seniors and families facing the unique challenges of Alzheimer's, dementia, or aging, in a place focused on safety, comfort, and personal care.
Bright hotel-like memory care excellent
I moved here for memory care and feel right at home - a bright, hotel-like, spacious facility that's very clean with chef-prepared meals and a fantastic dining room. The low-turnover, well-trained staff are caring, attentive and knowledgeable about Alzheimer's, run extensive activities and daily walks, and engage families; service feels truly 5-star. They can be busy and could use more help at times, but they give 200% - I highly recommend this place.
