    Inconsistent, neglectful care; unsafe conditions

I placed a loved one here and had mixed feelings: a few staff (Joyce Ellis especially) were warm, therapy and activities were good, and the grounds/lobby looked nice. But overall care was inconsistent and understaffed - dirty, cold rooms, missed meds and monitoring that led to pneumonia/bedsores, unanswered call lights, lost personal items, cold/inedible food, poor communication and a prison-like locked unit. It was a scary, exhausting experience and I would not go back for long-term care.

About Hebrew Center For Health And Rehabilitation

Hebrew Center For Health And Rehabilitation sits at 1 Abrahms Boulevard in West Hartford, where you'll find a range of health and rehabilitation services for adults age 50 and older, and you know, it's a big place with 257 beds, so you won't feel like the only one there. People go there for both short-term rehab and long-term care, and the staff includes lots of different specialists like doctors, nurses, physical, occupational, and speech therapists, so you can get care for many types of health situations including therapies, IV treatments, wound care, and even VAC therapy. There's an Adult Day Center, too, where seniors can go during the day for socializing and medical help, and the place shares space with a Connecticut Behavioral Health Hospital for those who need mental health services along with their regular care, and there's a Connecticut Geriatric Specialty Group too, where you can get more specialized check-ups, like memory screenings, and Hoffman SummerWood is part of the campus, though people don't usually talk much about what goes on there specifically. The Memory Care area stays secure to help folks who need more supervision, and there are always activities being organized-full-time activity directors look after that side of things, while social workers and therapists help out too. You're also likely to see things like daily events, therapy sessions, and maybe even telehealth visits or hospice care, since the place uses an approach called Passport™ to match what each resident really needs, whether that's home care or diagnostics. The place opens up for visitors every day from 8am to 8pm and even offers virtual visits if that's better. The Center works as a non-profit, provides financial help for those who need it, and tries to keep a warm, healing feel so folks and their families get the care and information they need. Leadership includes a President and CEO, a Medical Director, and a Director of Nursing, and there's a dedicated role for safety and quality. Hebrew Center For Health And Rehabilitation connects to other health services through National Health Care Associates and belongs to the Connecticut Association of Health Care Facilities. Over the years, the place has gotten some recognition, like a 5 Stars Quality Measure from CMS and the McKnight's Excellence in Technology Award for its use of Circadia Health. Staff do their best to treat folks with compassion and respect, focusing on honesty and real support, and the place has even been named a Certified Great Place to Work.
