Aspire at Evans is a care community in the area with 120 certified beds and an average of about 108 residents each day, and while the facility covers a wide range of services like independent living, assisted living, memory care, home care, and skilled nursing, it's also a place people go after hospital stays for rehabilitation, stroke recovery, or therapies like physical, occupational, and speech, which means there's a lot of medical support but staff time is 3.53 nurse hours per resident each day, falling a bit below the state average of 3.9, and nurse turnover is higher than most at 47.1%, which sometimes makes things feel a little unsettled compared to the state's 43.6%, so some consistency in care could be a challenge though the staff and caregivers are there for help with bathing, dressing, medication, and moving around, and the 24-hour call system and regular supervision provide a sense of safety, with English-speaking caregivers there for daily needs. The place is managed by Aspire Mgt LLC and Nochum Freund since November 2023, under the wing of Aspire Health Group, and though it holds strong community ties and several accreditations, inspection reports have counted 33 deficiencies, including one infection-related issue, plus a reported fine of $20,209 for things including infection control problems and concerns like not always providing enough food or fluids. They've built a reputation in the area and have a "people-first" approach focusing on choice and individualized care, offering everything from daily activities in the game room or activity room to movie nights, music, and even resident-run programs, and there's an outdoor area with walking paths, places to sit, and lots of scheduled activities to help people stay engaged. Meals here come from a chef, and residents can choose private rooms with kitchenettes, cable TV, and air conditioning, with flexible meal options for allergies and diabetes, plus showings in the movie theater, a library, spa room, fitness space, and family support services to help with moving in or staying connected. Even with the full range of services from long-term nursing care to home health care and hospice, Aspire at Evans has regular care assessments and support plans for each person, aiming to keep everyone healthy and supported, but the deficiency history and above-average nurse turnover are things to keep in mind for anyone considering the community, since they show that it's still working through some ongoing improvements even with all its services and amenities for residents and their families.
Neglect, poor hygiene, medication errors
I placed my loved one here and it was a terrible experience overall. Staff were slow or non-responsive - long call-button waits, unhelpful CNAs, nurses often absent, short-tempered and poor communicators. Meds and allergies were mishandled, orders ignored, and hygiene/safety was awful: dirty floors, debris, cockroaches, clogged toilets and even missed glaucoma meds and unemptied urinals. Rehab had some bright spots (excellent PTs and a caring speech therapist named Lauren), but therapy was inconsistent, too short, and they pushed us out early while my loved one still couldn't walk. Communication was nonexistent - no updates, rude front desk, after-hours phone access poor - and I feared for the patient's safety (others reported worsening, pneumonia, or death). A few staff were kind and the dementia wing/therapy sometimes helped, but the cleanliness, staffing, medication errors, and neglect risk mean I cannot recommend this place.
