    Unsafe, unresponsive staff; avoid facility

I would not trust this place with a loved one. My family saw chronic poor responsiveness-ignored call lights, inattentive front desk, patients left in the same position for hours/days-and unsafe care (missed meds/diabetic diet errors, privacy breaches, missed suctioning, inadequate equipment) that led to hospital transfers. Staff are often rude or overworked (weekends worse), communication is poor, and billing/management problems persist. That said, a few caregivers (Sharonica, Natasha and some CNAs), Steven, Evyonne and the rehab team were compassionate and effective, and rooms/therapy can be good. Overall the safety and consistency issues outweigh the positives; I would look elsewhere.

About Douglasville Nursing And Rehabilitation Center

Douglasville Nursing And Rehabilitation Center sits at 4028 Highway 5 in Douglasville, Georgia, where it's been operating for over twenty years, and the place always feels busy with plenty of staff on hand, since the number usually ranges between 201 and 500 employees for its 246 certified beds and daily census around 221 residents, so there's plenty of activity to watch and people working throughout the day and night, though you'll notice the nurse turnover rate reaches 55%, which is higher than the state average of 48.8%, but there are still licensed nursing staff offering skilled nursing, long-term care, palliative care, and short-term rehabilitation, along with respiratory therapy and outpatient services, with modern therapy equipment available and all sorts of therapy options, like physical, occupational, and speech therapy, for injuries or recovery after surgery, or therapy for things like stroke, voice disorders, swallowing difficulties, or cognitive problems, and even some advanced programs, like H2O SIPS, FEES, Wii-habilitation, Wheel SMART, and enhancement therapy for wound care, neuromuscular issues, or right after orthopedic procedures, plus there's a special Pulmonary and Ventilator Unit where staff trained in breathing care, including travel respiratory therapists like Anthony Cooper and M.Lee Andrews, work closely with residents who need help with ventilator or tracheostomy management, and a pulmonologist visits every week, so the center seems very focused on complex medical needs.

You can tell they try to keep everyone comfortable and active because there's a large activity room where something's always going on, a courtyard outside for fresh air, chapel and beauty shop inside, modern rehab suites and private rooms with TV, cable, Wi-Fi, phone lines, and private restrooms, along with wide hallways, clean personal closet space, and friendly housekeeping and laundry services, while meals get served in what they call a 5-star dining experience, and there's even a Right Way Café for snacks or different dietary needs, and if someone needs to get to appointments, the center has a wheelchair van, with a special separate entrance and renovated suites for those in the post-acute rehab program called The Suites.

Some folks stay only a short time after surgery or illness because short-term rehab is a big part of what goes on here, with therapy six days a week in the big therapy gym, and some folks are longer-term because they need ongoing skilled or assisted living care and someone to help with things all day and night. Staff work up care plans that address each resident's individual needs, though the inspection records show 41 total deficiencies in recent years, including two about infection control and some problems with medication management in the pharmacy, so it looks like there's always room for improvement, but staff do provide case management, discharge planning, and on-site diagnostics, plus a lot of help for families through psycho-social support and education.

There are visiting hours every day from 8:00 am to 8:00 pm, so families usually find it easy to come by, and the website for the center posts pictures of the spaces, along with details on all programs, events, and photos of staff, including people such as Betty Sheer, who manages the business, and other professionals mentioned in their official material, like LaBrita Cash-Baskett, and Marlow Butler from Legacy Healthcare Services. Douglasville Nursing And Rehabilitation Center is owned 50% by Dtd Hc, LLC, and 50% by D&N, LLC, and it's officially categorized as both a Nursing Home and a Health and Wellness Facility, so it's designed with hospital-level care in mind and a focus on both medical and daily living needs, and though the place isn't accredited by BBB, it's been part of the community since 2006 under the current business file and seems to offer a wide variety of services for seniors who need anything from basic care and companionship to complicated medical treatment or rehab.
