    Compassionate staff but unsafe, unclean

I moved my mom here because it's close and many staff-especially the nurses and some CNAs-are caring, attentive, and knowledgeable; therapies and visiting hours are convenient and management can be hands-on. However staffing is inconsistent and often overworked, causing long call-bell delays, poor coordination with doctors, medication/wound-care lapses and uneven management. Cleanliness and safety are my biggest concerns: persistent urine/filth odors, soiled linens/rooms, pest and mold reports, unsafe rails and occasional resident-safety incidents. Meals, activities and responsiveness improve at times and leadership has addressed issues, but overall I'd recommend cautiously and only if you can stay very involved.

For long-term residents, about 84% maintain their ability to move, eat, and conduct common activities without assistance, suggesting effective support for those able to retain a degree of independence.

The facility places importance on preventive care practices, with ongoing efforts to ensure residents receive vaccinations and appropriate infection control. Approximately 45.4% of long-term care residents have received their annual flu vaccinations. Grove of Berwyn provides personal and medical assistance to those unable to perform activities of daily living and endeavors to meet professional standards in all aspects of care. The facility has experienced periodic state health, fire, and safety inspections, which are carried out at least every 15 months. Some of these areas have been cited for improvements, but measures have been taken to address and resolve the identified concerns.

The environment at Grove of Berwyn is structured to prioritize resident wellbeing and safety, and protocols are in place to prevent hazards and ensure that each individual is treated with dignity and respect. The staff adheres to policies to prevent abuse and neglect and to report and investigate any suspected incidents promptly. In addition to clinical care, the facility promotes an environment where residents can maintain as much independence as their health allows.
