Community Care Center offers short-term rehabilitation and long-term mental health care, aiming to help people recover and return to their previous level of functioning whenever possible, and if that isn't possible, there's always a strong focus on comfort and quality of life; the facility provides memory care services for residents with Alzheimer's or dementia-related illnesses and features a special care unit for long-term mental health needs along with individualized care plans tailored to each resident's needs, all carried out by staff who are dedicated to individual households and encouraged to join residents at meals and activities, which helps everyone feel like part of a tight-knit community. Residents have access to 24-hour nursing care, skilled nursing and rehabilitation services, medication management, assistance with daily living, and outpatient physical, occupational, and speech therapies available six days a week; the rehabilitation gym is fully equipped with state-of-the-art therapy equipment for stroke rehabilitation, wound care, and retraining after injuries or hospitalization, and clinicians have a focus on enabling residents, when possible, to return home after their rehab is finished, all while palliative, respite, and non-ambulatory care are also available. The facility holds a non-profit status, operates as part of a Continuing Care Retirement Community (CCRC), accepts Medicare and Medicaid insurance, and works to support people with more serious medical needs after a hospital stay, illness, or surgery, ensuring support is there whether someone needs long-term care or a brief stay for recovery. Residents at Community Care Center benefit from amenities like private and semi-private suites, private rooms with private bathrooms that open onto each household's shared kitchen, dining, and living spaces, and beautifully landscaped grounds with walking paths, residential outdoor spaces, front porches for outdoor activity, gardens, and an outdoor common space, with a warm and supportive atmosphere meant to feel like home. The facility provides daily housekeeping, laundry, move-in coordination, salon services, fitness centers, a rehabilitation gym, a movie theater, arts room, music programs, scheduled daily activities, enrichment programs, social activities including trips and church services, and a café where residents and guests can enjoy coffee, cappuccino, beverages, or soft serve ice cream anytime. Residents have Wi-Fi, high-speed internet, cable TV, air conditioning, kitchenettes, telephones, and access to a concierge for various needs. The staff offers a holistic approach with programs designed to support social, physical, and emotional well-being, always keeping the focus on maintaining independence where possible and improving quality of life, providing transportation and parking, podiatry services, professional chef-prepared meals including special dietary options, emergency alert systems, and allowed family councils to help shape the care and environment for the community. There are four households within the center, each designed to foster connection and person-centered care, with about 120 certified beds (9 open as of June 2025), and a team that commits to compassionate, individualized attention, always working to meet each person's unique needs and support community involvement for everyone who lives there.
