    Pine Knoll Nursing Center stands in Lexington, Massachusetts, surrounded by woods, parks, and farms, with tree-lined courtyards and porches where folks can sit and watch the day go by. The facility has 81 beds, including private, semi-private, and four-bed rooms for both men and women, all licensed by the Commonwealth of Massachusetts and open to Medicare and Medicaid. Pine Knoll offers both short-term rehabilitation for folks who need to recover from surgery or illness, and long-term skilled nursing for people needing more daily help, with a staff available 24 hours a day, 7 days a week. Staff there handle nursing tasks, incontinence care, high-acuity care, wound care, IV medications, and help with things like bathing, dressing, eating, and moving around.

    You'll find specialized services such as podiatry, dentistry, psychiatric care, and optometry along with therapies like physical, occupational, and speech to help with things like talking after a stroke or getting back strength and balance. They also have support groups and counseling for residents and families, hospice for end-of-life needs, and respite care to give caregivers a break. There's a focus on helping folks stay independent and their programs include games, fitness classes, music therapy, outings, and regular activities aimed at improving quality of life. Three meals a day come in a nice dining room, and there's a library, common rooms, laundry, and indoor and outdoor areas where you can gather with friends or family. There's also high-speed internet and some pet-friendly policies for people who want to have a cat or dog with them.

    For medical needs, Pine Knoll works closely with physicians and nurses, offers pharmaceutical, radiology and lab services, and has someone to help with transitions between different healthcare settings. The facility works on culture change to make the place feel more like a home and tries to cut down on unnecessary antipsychotic medications while running fall-prevention programs, aiming to keep people safe.

    There are real concerns, though, and Pine Knoll is on the list as a Special Focus Facility Candidate due to a history of serious quality issues. In September 2024, inspectors found 35 deficiencies, three related to infections, and Pine Knoll's had a large fine, totaling $214,636, for violations connected to nursing, physician care, administration, and overall quality of life and care. Staffing levels, with about 3.65 nurse hours per resident per day, fall below the state average, but nurse turnover is a bit lower than most. Management has included Asif Merchant since 2014 and Matthew Sweeney and Thomas Woods since 2018, and the group running the place, Long Term Centers of Lexington, Inc., has plans to expand and upgrade in the coming years.

    Altogether, Pine Knoll Nursing Center delivers a wide mix of services for older adults, caring for those who need short-term rehabilitation, skilled nursing, or extra support as they age, set in a warm location with leafy views, but it also carries a record of past quality concerns that families and future residents should weigh along with its offerings.
