    About Ebenezer Care Center

Ebenezer Care Center, also called Ebenezer Integrated Care & Rehab, is a non-profit nursing home and senior living community run by the Ebenezer Society since June 2011, and it's got 62 certified beds with a range of care options like independent living, assisted living, memory care, enhanced care, transitional care, skilled nursing, home care, hospice care, and even adult day programs like Ebenezer DayBreak in St. Paul, so there's something for all sorts of needs and stages of aging, and they use special names for their care units and programs to match each resident's needs, but it's important to know that inspection reports have found issues over the years, including problems with quality of life and care, handling of medications and treatment according to resident preferences, proper infection control like vaccination policies, and making sure special dietary needs are met, and these led to a federal fine of $26,685, and although the nurse turnover rate's a bit higher than the state average at 46%, they actually provide more nurse hours per resident each day, at 5.61 compared to the state's 4.2, so there is solid staffing throughout the day and night, with nurses on 12 to 16 hour shifts and a 24-hour supervision and call system, and the staff includes registered nurses, licensed practical nurses, nursing assistants, housekeepers, and activities assistants, all working together with therapists, social workers, dietitians, and chaplains for an all-round care approach, and in terms of the setting, there are private rooms with their own bathrooms, kitchenettes, cable TV, air conditioning, Wi-Fi, and rooms come furnished, plus the physical campus has outdoor spaces, walking paths, gardens, lounges, and community rooms where folks gather for music, games, storytelling, special outings, lifelong learning, fitness, and other group and family support activities, and meals are prepared by a professional chef, with all-day dining, restaurant-style service, and accommodations for allergies and dietary restrictions.

The adult day program serves up a noon meal and snacks, daily exercise, monthly health checks, and provides time for caregivers to take care of other needs, while the on-site pharmacy consults, dietitian services, spiritual care by the Ebenezer Chaplain, and the Hub Café and Market give both medical support and comfortable conveniences, and there's free guest Wi-Fi, secured access, open visiting hours, and spiritual health supports, not to mention activities and entertainment led by creative, energetic staff and opportunities for seniors to form friendships and socialize in a cheerful space, while families have peace of mind knowing there's quick help for things like bathing, dressing, medication management, transfers, fall prevention, and specialized care for folks with memory loss through a dedicated memory care unit with controlled access.

The rehabilitation program provides therapies five to seven days a week-physical, occupational, and speech-to help with recovery after hospital stays or illness, while health services extend to IVs, wound care, pain management, nutritional counseling, ultrasound and E-stimulation therapy, and certified lymphedema treatments, plus a 24-hour call system, which works alongside the variety of community-sponsored activities, scheduled daily events, and even resident-run initiatives to keep everyone active and engaged. Residents and families can access parking at a reduced rate and pay through Medicare, Medicaid, private pay, long-term care insurance, VA, Elderly Waiver, and Alternative Care Grants, and the organization has a mission to help those in need, including low-income seniors with affordable housing. Inspections show there's room for improvement in infection control, nutrition, and honoring resident choices, but the community keeps a focus on healing and helping seniors discover longer, healthier, meaningful lives in a diverse neighborhood close to clinics, shops, and parks, and all these services run Monday to Friday, 8:00 a.m. to 4:00 p.m., except major holidays, for those coming for adult day programs.
