    Poor care, neglect, infections, avoid

    I moved my mother in hoping for better care, but promised changes never happened. Care was poor-long nurse-call waits, delayed pain meds, broken/missing wheelchairs and lifts, ineffective PT-and her health declined with infections and bedsores. The room was foul-smelling, dirty and cluttered in an outdated building with tiny shared bathrooms; meals were unappetizing, very small, sodium-heavy and the diet never changed. Some aides and cleaning staff were helpful and staff kept family informed with quick COVID tests and physician checkups, but overall it felt neglectful and not worth the money. I do not recommend this facility.

The facility works to support residents' personal routines and gives each person help with daily needs like bathing, dressing, medication, moving around, and meals while making sure skilled nurses and staff are around all day and night for medical care, supervision, and emergencies, and when someone needs help with complex care like tracheostomies or physical, speech, or occupational therapy, those services are available, too. There's training for staff covering topics like Dementia & Alzheimer's care, Medicaid, and RAL courses, with state caregiver certifications, ongoing staff education, and policies to prevent abuse, plus careful screening for employee backgrounds and keeping surety bonds for resident funds, and though some staff files have missing background checks, the facility's supposed to check the Family Care Safety Registry and Nurse Assistant Registry before hiring.

    Residents can relax in a place filled with homelike touches, private bathrooms, cable TV, kitchenettes, air conditioning, and high-speed internet, and rooms come in different sizes. For activities, there are arts and crafts rooms, a fitness room, a library, a spa with a sauna, outdoor gardens, walking paths, business and activity rooms, a movie theater, and common spaces where people gather, as well as programs like movie nights, music, outdoor programs, and resident-led events. The dining room offers meals made by a professional chef with options for people who have diabetes or food allergies, and a dietitian oversees nutrition. Transportation helps get residents out and about, and there's parking for visitors.

    Housekeeping, laundry, move-in help, spiritual and family support programs, and salon services are all available, and nurses, a nutrition specialist, and therapy staff provide hands-on support. The community follows strict policies for medical orders, keeping careful records, and giving proper tracheostomy and infection care, and it accepts both Medicare and Medicaid. Lincoln County Nursing & Rehab's ownership is for-profit, it's not a CCRC (Continuing Care Retirement Community), and though the building is large, the staff aims to keep things homelike and respectful of each resident's needs, abilities, and wishes when planning daily life, so people can feel safe, cared for, and part of a community, even when they're facing serious health concerns.
