    Attractive facility, inconsistent care, unsafe

I liked the clean, nicely furnished apartment, common areas, salon, exercise and PT room, and generally good meals and activities. In-house PT/OT is available but was inconsistent - therapy was sometimes promised and not delivered. Staff were a mix: some kind and helpful, others rude, lazy, or on their phones; I don't trust the social worker. Serious care problems worried me: delayed responses to complaints, port bandage left risking infection, breakfast with hair, restrictive/confined treatment and even being held against family wishes. Cell phone reception is terrible. Overall the place looks great, but inconsistent care and management issues left me dissatisfied and concerned.

About Southlake Village Rehabilitation & Care Center

Southlake Village Rehabilitation & Care Center is a skilled nursing facility with 126 certified beds, located in a place that sees about 116 residents each day, and you'll find that it's owned and managed by Vetter Senior Living and Vsl Vetter Health Services LLC, with people like David Bergmann, Brian Stuhr, Ted Triggs, Glenn Vanekeren, Eldora Vetter, and Jack Vetter involved in management since 2017 and 2018, and being a non-profit, the facility accepts both Medicare and Medicaid. Folks have access to round-the-clock supervision and personal care, so whenever someone needs help with bathing, dressing, transfers, or medication management, there's always certified nurses and nursing assistants on hand, and the nurse turnover rate sits at 37.1% which is lower than Nebraska's average, and nurse staffing's a bit higher than what you see around the state, with about 4.96 nurse hours per resident each day. Residents get private bathrooms, kitchenettes, cable TV, Wi-Fi, furnished rooms, air conditioning, and telephones, and there's a 24-hour call system, which means someone's always available when needed.

The facility does offer quite a few extras, with housekeeping and laundry, move-in coordination, and family support services, as well as transportation, parking, and a range of common areas like a library, movie theater, arts and activity rooms, music programs, a game room, outdoor patios, walking paths, a garden, and a fitness room, plus various scheduled activities every day. There's community and resident-run programs, outdoor programs, spa and wellness areas, and organized movie nights, which means folks often have something to do if they want to stay busy or active, and the dining room has all-day dining with a professional chef that prepares meals for special diets, allergies, diabetes, and such.

On the healthcare side, Southlake Village provides skilled nursing, long-term care, short-term rehabilitation, sub-acute care services, hospice, and respite care, along with Alzheimer's and Dementia programs. The staff's trained to address needs after a hospital stay, illness, or surgery, with therapy services like physical, speech, and occupational therapy, and restorative programs. They put emphasis on health and wellness support, personalized care plans, and helping residents regain daily skills, with technology that lets physicians reach records remotely, do real-time charting, and track information for better oversight.

The facility has had a total of 16 deficiencies in inspection reports, with two related to infection control, including a failure to maintain proper infection prevention standards and correct medication storage practices. One recent inspection identified pharmacy and infection control concerns that could potentially result in more than minimal harm. Although the nurse coverage and hours are better than many similar places, these citations do stand out in state reports.

Residents who have mobility limits can get non-ambulatory care and transportation services, and supervised support means there are security protocols and safe, controlled environments for folks that might wander or need extra monitoring. The team includes certified nurses, qualified nursing assistants, licensed medical staff, and physical therapists, all working together to provide comprehensive care tailored to each person's needs. Southlake Village puts focus on helping residents manage both health and daily living, offering comfort and essential services as needed, though publicly available information can sometimes have gaps due to server errors or incomplete data. Southlake Village remains dedicated to quality care, with a full set of programs and amenities, while the latest inspection reports give families important facts to consider.
