Four Seasons Nursing & Rehabilitation Center sits at 1555 Rockaway Parkway in Brooklyn, NY and takes care of people needing skilled nursing and rehabilitation. People come here for help with stroke, heart problems, wound care, and breathing support, and the place is led by a board-certified pulmonologist who oversees ventilator and respiratory services. The ventilator unit, which has run for over 20 years, is New York State Certified and has a strong record for helping people come off ventilators. Staff help with all kinds of medical needs, using individualized care plans. On each floor, there are residential lounges, and rooms are single or double, each with a private bathroom and shower, along with individual climate controls, and they've been recently updated.
There's always a licensed nurse close by, and there's a high nurse-to-resident ratio, so people usually don't have to wait long for help. Physical, occupational, and speech therapists work with residents, focusing on recovery from surgeries or illnesses, and offering chronic disease management, pain management, medication checks, and post-operative care. Most residents use wheelchairs and need full care with daily tasks. Group and individual activities are part of daily life, along with health classes, religious services, holiday parties, and a beauty parlor/barber service on site to help with grooming.
Residents can join special events, and the center offers support for a variety of lifestyle needs. Meals come from New York State registered dietitians and can be eaten in the dining room, which tries to offer culturally diverse and gourmet options, but sometimes food comes out cold, which people have criticized. Wi-Fi and phone service are free, and every month, the center updates its online provider directory. English is spoken by staff, but it's not clear if other languages are supported. Four Seasons is currently not taking new patients.
