Park Terrace Care Center is a 200-bed facility dedicated to providing 24-hour comprehensive medical, nursing, and rehabilitative care. This center places a strong emphasis on individualized care, ensuring that each resident’s unique needs are met through a range of specialized services and programs. With a focus on residents recovering from brain injuries, the care center has developed a Certified Brain Injury Program, which is recognized by the New York State Department of Health. This program offers intensive rehabilitation in Physical Therapy, Occupational Therapy, Speech Therapy, and Cognitive Therapy to support the recovery and long-term well-being of brain injury patients. Daily programming at Park Terrace Care Center also includes memory and orientation groups, cognitive and recreational activities, yoga, and art therapy, providing residents with a holistic and engaging approach to rehabilitation.
The Certified Traumatic Brain Injury (TBI) Unit serves as an intermediate rehabilitation phase bridging the gap between acute hospital care and full community reintegration. Residents are encouraged to remain active, and unlike traditional hospital settings, those enrolled in the Certified TBI program are expected to be out of bed and dressed each morning, with attention given to clothing style and comfort. The care center provides a laundry service and labels each resident’s clothing for ease and organization. Family involvement is highly encouraged, especially outside of therapy times, as research has shown that supportive family presence can positively affect recovery outcomes. Residents and their families are invited to personalize their rooms with tasteful items and family photos, which can help with orientation and foster a comforting environment.
Medical oversight at Park Terrace Care Center is comprehensive, with an on-staff medical director and on-site consultations available for specialties such as Physiatry, Psychiatry, Pulmonology, Wound Care, Podiatry, Ophthalmology, and Dentistry. This multidisciplinary team works collaboratively to address each resident’s complex rehabilitation and healthcare needs. The rehabilitation process at Park Terrace Care Center is individualized, recognizing that every brain injury is unique and that residents respond differently to therapy. The goal is always to maximize independence, with most residents working toward returning home with or without home care services, or transitioning into outpatient rehabilitation programs. For those facing more significant or chronic disabilities, or who lack sufficient community resources, the center offers a long-term “Extended” TBI program.
Admission to the program is available for patients aged sixteen and older who have sustained structural, non-degenerative brain injuries and are medically and psychiatrically stable. Most patients are referred from acute hospital rehabilitation settings, coming to Park Terrace Care Center for continued progress in a nurturing and supportive environment. The center’s short-term rehabilitation program is well-suited for individuals who require intensive services over a shorter period before returning home. Families navigating admissions and financial arrangements can count on dedicated staff, including a full-time Medicaid Coordinator who assists with the often complex process of Medicaid applications and eligibility.
Park Terrace Care Center is proud of its achievements, highlighted by a 4-star rating with CMS for quality measures and a 3-star rating for staffing. The center’s commitment to maintaining high standards in clinical care and quality of life is evident in its thorough and compassionate approach to rehabilitation and long-term care. Residents continue in their programs as long as rehabilitation goals are being met, for up to one year, with the aim of enabling each individual to achieve the highest possible level of independence and community participation.
