Both a social worker and a case manager are involved in guiding new residents and their families throughout the process. Upon admission, an individualized care plan is developed for each resident, emphasizing the restoration of independence and ongoing improvement in quality of life. This care plan is crafted by the interdisciplinary team and is regularly reviewed and adjusted as needed to ensure optimal outcomes for each resident. Discharge planning is initiated on the day of admission to ensure a smooth transition and continued support.
