    Clean facility, neglect when unattended

I found the building, rooms, dining area and therapy/activities nice and clean, and some staff (including therapy and a few nurses/CNAs) were caring and accommodating. But chronic understaffing, high turnover and poor communication made care uneven - when family advocated my relative got good, attentive care; when we weren't there I witnessed neglect (missed hygiene, soiled bedding, ignored dietary restrictions, delayed wound/medication attention and even bedsores). I also dealt with missing items, slow/unhelpful social work and inconsistent medical oversight. Administration can be responsive at times, but I would only consider this facility if a family member can be present and advocate daily; otherwise I cannot recommend it.

    About Crestview Court

Crestview Court is a skilled nursing facility with 125 beds, offering a wide range of care for seniors needing long-term support or short-term rehab in a setting that feels calm and upbeat, almost like home, with staff who are known for being warm and helpful such as activities director Yolanda making sure people stay active and social. The building offers studio and semi-private rooms, including electric beds, and you'll see both men and women living there. Meal plans focus on balanced, tasty food served three times a day, and there are beauty and barber shops on site. Residents can join activities both inside and outside the building, which helps them stay connected and active. For those needing memory care, Crestview Court provides specialized housing and therapies designed to help people with dementia and Alzheimer's disease feel safe and supported, which everyone seems to appreciate because it helps prevent confusion and wandering.

People who can live mostly on their own might use the independent living services, where they get a hassle-free social lifestyle, but for folks needing more help, there's a good range of assisted living, home care, respite stays, skilled nursing, and hospice care right there, so you don't have to move again if your needs change. There's rehab with physical, occupational, and speech therapies for those recovering from surgery, stroke, or injury, and the facility offers addiction medicine, pain management, and specialized physiatry medicine so seniors can get comprehensive treatment without leaving the building. Doctors see patients twice a week, with services offered every day, around the clock. Crestview Court has pharmacy services open seven days, and new patients get their necessary medicines when they arrive.

There's discharge planning help, nutritional support with selective menus and therapeutic diets, lab work, and X-rays done on site, with outpatient rehab options too. Staff handle medication management, IV therapy, respiratory therapy, wound care, trach care, hygiene assistance, and personal care, while the management team and socials services help with transitions and day-to-day issues. Seniors can expect 24-hour skilled nursing, regular recovery programs after heart or lung problems, chronic conditions, or surgery, and help for families when their loved one needs end-of-life hospice or palliative care.

Crestview Court holds a 3-star rating from the Centers for Medicare & Medicaid Services and a community score of 7.1 out of 10 from resident reviews, which means families and seniors recognize that it's a good, solid place with supportive amenities and staff, but it's not perfect. The environment aims to be comfortable and homelike while providing all the nursing care, therapies, and daily support older adults might need as they age.
