    Understaffed facility with poor management

I have a family member here. The building and lobby look nice and some nurses and therapists are very caring, but overall the place is understaffed with high turnover and an uncaring administration. Call lights and requests are often slow or ignored, meals are limited and inconsistent, rooms are small/dated, and cleanliness and basic dignity (urine odor, dirty bathrooms, inadequate shower assistance) are recurring problems. Communication from management is poor, though a few staff have been responsive after I raised issues. My dad is mostly happy, but I would not recommend this facility without major staffing and management improvements.

About Forest Park Nursing & Rehabilitation

Forest Park Nursing & Rehabilitation, located at 6825 Harry Hines Blvd in Dallas, TX, is a skilled nursing facility that offers both short-term rehabilitation and long-term care for residents, and you can tell they've thought about healing with their range of physical therapy, occupational therapy, speech therapy, memory care programs, and post-operative rehab, so people coming here often have choices in care and services, and they offer 24-hour skilled nursing and physician-supervised care aimed at helping folks stay as healthy as they can. The facility has 150 certified beds, usually averaging about 103 residents a day, with private rooms that have TVs and phones, plus high-speed internet so residents can stay connected; there are indoor and outdoor common areas too, including a nice patio garden, and amenities like an on-site beauty salon, a barber shop, laundry and dry cleaning services, and regular activities, including some that happen off-site. You'll find an experienced team working here, with roles like MDS Nurse, Occupational Therapy Assistant, and Registered Nurse, and they design individual care plans with outside board-certified physicians and even coordinate Home Health if residents need to safely go back home, and for those with wounds, they offer wound care and Vacuum-Assisted Closure therapy. There's a focus on helping residents meet their goals with supervised therapy and meals reviewed by a certified dietitian, plus social services to help during transitions or longer stays, so they try to keep things comfortable and supportive.

But you should know the facility has had some issues over the years, being a Special Focus Facility Candidate-which means it's had a history of serious quality problems, though not yet officially flagged by the government, and the current management, Forest Park Nursing & Rehabilitation LLC, took over in November 2022, with David Garetz as the administrator, and the Dallas County Hospital District has been involved since 2017, while Fundamental Healthcare is listed as an affiliate, so there have been a few hands in the management pot. Inspection reports show 53 total deficiencies, including serious problems like not reporting suspected abuse, neglect, or theft, failing to make and follow complete care plans, and once having an area deemed an immediate hazard to residents' safety; there have also been two infection-related deficiencies and other concerns about stopping the spread of infections in the building. The nurse turnover rate stands at about 67%, with about 2.88 hours of nurse staffing per resident each day, so staff changes seem pretty common-something people might want to keep in mind if consistency is important. Still, the facility says it's committed to a respectful, caring environment focused on physical, social, and emotional well-being, and staff work to help residents regain independence whenever possible, using individualized rehab programs, palliative care for those who need it, and attention to comfort and dignity; you'll even find activities tailored to each person's interests to help quality of life. It's a privately held company with a small social media following, and there's a real effort to pay attention to each resident's unique needs, but with the inspection history and frequent staff turnover, families may want to ask more questions to make sure Forest Park's environment is the right fit for their loved ones.
