    Neglectful, unsafe, unresponsive dementia care

I placed my loved one here and deeply regret it. Staff were neglectful and only attentive when management was around-Manager David seemed hardworking, but the team lacked urgency and often didn't return calls or notify family. The secure dementia unit effectively ignored residents: call lights went unanswered for long stretches, patients screamed, and I saw infections and issues that felt preventable. The building smelled of ammonia on entry, rooms for two were small and moldy, there's no real care/therapy programming, and though it looks kept, it's old and unsafe-do not send your loved ones.

About Desoto Long Term Care Partners

Desoto Long Term Care Partners, also known as Millbrook Healthcare and Rehabilitation Center, sits near DeSoto, Texas, and is part of the Gulf Coast Long Term Care Partners group, offering a range of care from independent and assisted living to memory care and skilled nursing, so you've got all the care options in one place, and when folks need help after a hospital stay or a big surgery, they've got rehab programs and services for that too, with therapy staff in-house to help with physical, occupational, and speech therapy, and people can count on wound care, IV therapy, and diabetes management, as well as help for those facing cardiac issues, strokes, or surgery recovery, which seems to cover a lot when needs change, making it an option for those who want to age in place. The community has a secure memory care wing for folks with dementia or Alzheimer's, and it runs 24-hour support, so safety stays a focus. Assisted living and nursing home suites are available, and with about 120 licensed beds (116 certified beds) but only 73 residents as of May 2025, there are usually open rooms.

Desoto Long Term Care Partners has Medicare and Medicaid certification and they take new patients, which is good to know since the Dallas-Fort Worth-Arlington area has a high demand for beds. Services like laundry, housekeeping, transportation, religious services, engaging activities, and even beauty and barber options are part of daily life, and residents can use the Payment Portal or Care Portal to stay connected with loved ones or manage bills. Medical care covers podiatry, optometry, dentistry, hospice care, medication management, and pain management, and there's diabetes and dietary help, too. The care team develops personalized care plans for each person, so needs get met, whether that's short-term rehab, wound care, long-term skilled nursing, help with daily needs, or support for memory care residents in a safe setting. The average stay is about 158 days, but there are a fair number of short stays, with 177 discharges and nearly 28,000 inpatient days for the last reported year.

When it comes to ratings, Desoto Long Term Care Partners puts up strong numbers, earning a four-star overall rating and five stars for health inspections and quality measures, placing it near the top for the area, with only one nearby facility scoring higher, although staffing sits below average compared to others in Texas and Dallas County, or the ten nearest homes, so there's room for improvement there, but health inspections and quality seem solid with recent surveys showing few deficiencies. Fines have been issued in the past, totaling $384,487 over the years, but there hasn't been a new fine in three years. The building is part of a larger chain, which can support good practices and consistent care, and with suites for different care levels and a balance of services for medical, social, and emotional needs, the community fits folks who want options as care needs change, without having to move again.
