    Chronic understaffing, neglect, poor leadership

    I placed my mom here because the building and rehab/PT looked great and a few CNAs and therapists were excellent. In practice it was chronically understaffed and unsafe - rude, neglectful nursing, long call-light delays, missed/late meds, falls, urine smell/soiled rooms and dirty bathrooms. Management was unresponsive, defensive and seemingly profit-driven; COVID and exposure communication was poor. Meals were often late/wrong and special diets ignored. I spent hours daily overseeing care and ultimately transferred her. Good therapy and a few caring staff don't make up for the ongoing safety, hygiene and leadership failures - hire home health or look elsewhere.

The place is set up to help people who need short-term rehabilitation after a hospital stay, as well as those who need long-term skilled nursing or intermediate care for major health needs. Residents get meals, personal care, hygiene help, and have access to both private and semi-private suites, with electric beds for comfort and both 20-inch flat-panel televisions and telephone handsets in the rooms. The nursing staff stays on site day and night, so people who need high acuity care or non-ambulatory support can get help right away, and there's a focus on making plans for each person using an interdisciplinary team.
