    Caring staff, but unsafe care

I placed my mom here and have mixed feelings: the therapy team, receptionists, and several nurses (including an amazing Director of Nursing) have been caring, professional, and personable, and the facility feels warm and family-like. But we've also experienced serious problems - chronic understaffing and high turnover, inconsistent/no showers (with alleged forged records), missed/delayed meds (including insulin), unanswered call lights, and nurses abandoning posts. Cleanliness and hygiene lapses have caused UTIs, missing towels/sheets and clothes, cold/late meals, and even reports of theft; some staff were rude or dismissive. Management responsiveness is hit-or-miss. After 30 days I rate it 2/5 - good people and therapy, but safety and basic care issues mean we will reevaluate.
