    Understaffed facility causing serious harm

    I found the facility clean, with friendly staff, clear explanations, and lots of activities - they cared well for my brother at times. However, chronic understaffing led to serious safety and hygiene failures: I witnessed neglect and resident injuries (including broken limbs), a nurse hit a patient, staff delayed or refused to call an ambulance, residents were left in their own waste, melatonin was given without consent, and privacy was violated. After I reported to state and corporate some things improved, but training and staffing remain inadequate; I cannot recommend this facility - avoid it.

    Golden Estates Rehabilitation Center is a nursing and rehabilitation community with 93 certified beds, run by Golden Rehabilitation Center LLC since May 2023, and affiliated with Hamilton County Hospital District, and it's the kind of place that tries to look after folks who need more care, from skilled nursing and short-term rehab to memory care and assisted living, so you'll see services for people with dementia or Alzheimer's, too, and home health care or even adult day care if that's needed. Staff members include administrators, nurses, activity coordinators, and people who supervise, and there's a Staffing Coordinator who manages the schedules, a Director of Activities who makes sure there's something to do every day of the week, whether it's social outings, group support, or engaging programs for residents-plus they have a Wound Care service by a licensed vocational nurse, with a Director of Nursing role coming soon, and a nursing staff that averages 3.26 nurse hours per person per day, though the nurse turnover rate is high at 60.3%. The center puts in effort to train caregivers through Medicaid programs, offers courses in Residential Assisted Living and senior home care, and runs dementia and Alzheimer's training; there's also help with state certification for caregivers. You'll find private or shared rooms, many with private bathrooms and their own furniture, and the place tries to keep things safe and accessible, thanks to controlled-access entries, emergency call systems, enclosed walkways, and paths for walking, and it does have outdoor courtyards with benches, trees, and even a playground for children if family comes to visit, all set up in big open spaces to make family gatherings easier. Golden Estates has a synagogue with an American flag, menorah, stained-glass windows, a Torah ark, and wooden pews, making it a special spot for worship or quiet moments. For amenities, there's a library, a beauty salon, a fitness center, a lounge, and restaurant-style dining, and you'll have access to on-site medical services like physical therapy, occupational therapy, speech therapy, and medication management, plus a 24-hour nurse coverage system if something comes up any time of day or night. The facility tries to support older adults to live as well as possible, and pays attention to comfort, safety, and well-being, with ombudsman services to help sort out complaints in private and at no cost, as well as auxiliary aids and language services for residents with disabilities or limited English, provided through interpreters and written translation when needed. While Golden Estates has a focus on compassion and quality, please note it has received citations for issues like infection control, resident rights, abuse reporting, and safety hazards in the past, so that's something to consider if thinking about staying there or placing a loved one; they're still committed to improvement and to giving elders as much comfort and support as possible during their stay.
