    Unsafe neglectful nursing and rehabilitation

    I appreciated the friendly, knowledgeable activities team (Bentley the therapy dog was lovely), nice rooms, strong housekeeping leadership and capable therapists - families often had to help out because staff were busy. However, nursing care was inconsistent and sometimes inattentive: delayed meds and medical evaluations, poor communication, inadequate monitoring (my mom fell several times), broken equipment, short/ineffective rehab, and meals that didn't meet special-diet needs. She was worse after rehab and ended up hospitalized; because of these safety, staffing, and compassion issues, I cannot recommend this facility.

    The Enclave offers memory care, skilled nursing, long-term care, respite care, and rehabilitation therapy under one roof, which means people can stay for short-term recovery or for longer periods when they need more help, and it's important to know that certified beds number 142 while the average daily residents come out to about 120, and the nurse staffing sits just above the state's average, coming in at 3.48 nurse hours per resident per day. There's studio and suite room choices, and a lot of rooms come with private bathrooms, large beds, and flat-screen TVs, plus some let you control the temperature, which helps people stay comfortable, and you'll find Wi-Fi throughout the building, as well as an internet café, so residents can keep in touch or browse online as they like. The dining options include daily gourmet meals, a home-cooked menu made by a registered dietitian, room service, or meals in the dining room depending on what someone wants that day, and there are also onsite salon services, a chapel for spiritual needs, and plenty of daily cleaning and linen services, which do help with keeping things tidy. For activities, there's an activity director, so you'll usually find art, gardening, outings, games, and music, all aimed at keeping people mentally and socially engaged, and they do therapy and rehabilitation in a modern gym with special pathways for physical therapy work. Security monitoring and transportation to doctors' appointments add to the overall safety and convenience for people living there, especially when families can't always make the trip, and touchstone's had managerial control since March 2023, aiming to prioritize family partnerships and faith within their care, trying to meet everyone's unique needs through personalized care plans. Along with support services like hospice, home care, and home health, The Enclave stands ready to help people bridge that gap between the hospital and home via short-term rehab, but it's also wise to note that inspection reports over the years show 37 deficiencies, including three for infection control, some for food safety and visitor food policies, as well as one about medication management, so there are areas needing improvement, even as staff work toward keeping up with regulations and caring for the residents as best as possible. Services extend to social media platforms, and a virtual tour's available for those who want a look without visiting, and for those needing help with discharge planning, that's part of the package, too. All in all, The Enclave brings together a lot of different services and amenities for people who need varied types of support, but, as with many places, it's important to look at all the facts, including the inspection reports, so families can decide if it suits their needs.
