Wellington Rehabilitation and Healthcare sits on beautifully landscaped grounds and offers both short-term rehab and long-term care, and the place uses special names for its facilities and services, which helps make things feel less clinical. The center has 124 certified beds with an average of 61 residents a day, and there are private and semi-private rooms that come with Wi-Fi and are well-kept by the housekeeping and laundry staff who are always around, making sure things are comfortable. Meals are nutritious and menus change to fit a variety of tastes, while a busy activities program lets residents enjoy everything from indoor events to excursions and outdoor activities, keeping folks active and connected, because staying busy helps pass the time and keeps everyone from getting too bored. Wellington provides different types of care to fit every resident's needs, including skilled nursing, catheter care, and incontinence support, and addresses things like urinary tract infection prevention as mentioned in their inspection reports, though there've been documented deficiencies in some areas like infection control and accident prevention, meaning they're working on these safety and quality issues as required by law. The nurse turnover rate stands at 75.8%, so residents may see new faces now and then, and the nursing staff spend about 3.03 hours per resident each day, which is pretty standard but something people should keep in mind if they need frequent help. In-house therapists use both modern equipment and hands-on therapy in state-of-the-art rehab rooms, helping residents regain strength and mobility so some can return home safely. The care plans result from meetings between staff, families, and outside providers to make sure care fits each person, and the team focuses on safety, therapy, and improving quality of life even while following federal guidelines and addressing issues from regular inspections. The facility's been under Wellington Healthcare, Inc. since April 2017 and is affiliated with The Ensign Group, which helps ensure continuity in how care and services are delivered.

Friendly staff; safety, overcrowding problems
I appreciated the friendly, professional staff, hospital proximity and accessible 24/7 care - therapists have long tenure and the Medicare-covered 20-day rehab felt very good. Activities (bingo, outdoor time, shopping trips, live music 2-3x/week) are busy and meals are homemade, warm and generally nutritious, though I worried about weight loss. Common areas and rooms looked clean and organized with comfortable beds and modern features, but the place sometimes smelled nasty and rooms were doubled up in tiny spaces. I saw ignored safety hazards (a loose sprinkler hose), inaccurate medication records, a rude nurse and an unconcerned DoN; staff can be short-handed and amenities are limited. Doors have alarms, but exits lead to a busy street and need better barriers. Overall care is largely competent, but safety, overcrowding and inconsistent staff attitude made me want to get my relative out.
