    Good amenities but care declined

I have mixed feelings: the facility is clean, convenient, offers excellent therapy and some very caring staff, single rooms and good value - but overall it has gone downhill. Too many caregivers are rude, pushy or distracted, complaints are swept under the rug, medications and personal care are often delayed or missed, residents have unexplained bruises, swollen feet, weight loss and soiled diapers. I would not return and strongly urge you to look elsewhere.

About Wesley Woods Health & Rehabilitation

Wesley Woods Health & Rehabilitation is a senior living community in Waco, Texas, offering nursing home care, long-term care, rehab to home, respite care, and memory care for people with Alzheimer's and dementia through the Wesley Woods Alzheimer's Care Center, where the staff gives 24-hour supervision and emergency support, and there's skilled nursing available for 12-16 hours daily, plus a 24-hour call system for resident safety, and the facility takes both Medicare and Medicaid for payment. The building has 120 licensed beds but offers 16 certified beds as of June 2025, with private rooms, bathrooms, cable TV, Wi-Fi, kitchenettes, and air conditioning, and rooms can be furnished or unfurnished depending on what the resident prefers. Most days see an average of 103 residents, and they enjoy various amenities like a movie theater, arts room, fitness room, garden, wellness spa, library, and spaces for group activities such as movie nights and music programs, so people always have something to do, whether it's art, gardening, cooking, storytelling, or playing games with friends. There's a country kitchen and cozy dining spaces, with a professional chef preparing meals, meal planners making sure food is nutritious, and staff offers all-day dining, allergy-sensitive meals, and special diets as needed, so residents and their needs around food are well taken care of, including private dining rooms for more intimate family gatherings.

Nursing staff handles medication management, bathing, dressing, help transferring, care for people who can't walk on their own, and personal care at all hours, led by a Director of Nursing and a clinical team, though the nurse turnover rate sits at about 56.7%, which is higher than the state average, although the nurse staffing hours per resident per day are also slightly above the state average at 3.76. Housekeeping, laundry, linen, and move-in coordination are part of the services, so residents have fewer chores to worry about. The community is divided into small neighborhoods with public spaces at the center, designed to give people safety, independence, and dignity, and the Alzheimer's care center keeps its programs and spaces secure and supportive for memory care, with cognitive activities and social events, companionship, spiritual support, outings, and counseling for both residents and their families.

Management includes Debra Thomasson and Adam Flowers since April 2023, Jack and Catherine Cook since December 2018, with ownership by Four Cooks Senior Care and control by Woodgate Senior Care LLC and Coryell County Memorial Hospital Authority; they have a team setup with an administrator, business office manager for financial help, an admissions and marketing coordinator, a social worker, and a lead nurse. The facility is for-profit and isn't part of a continuing care retirement community, but does offer structured schedules with a steady flow of social, educational, and entertaining activities to promote health and connections. It's located near hospitals, doctors, and local entertainment, so getting medical attention or outings isn't hard, and the community environment helps residents feel less isolated and enjoy a sense of independence through the many group programs and support services.

While Wesley Woods has many amenities and services, inspection history does raise concerns, with a past elder abuse citation, 24 total inspection deficiencies, some infection deficiencies, and, more recently, resident rights issues found in March 2025 about honoring dignity, communication, and residents not always being fully informed about their health status, but inspectors did not find actual harm had occurred, only the potential for more than minimal harm. Staff at Wesley Woods tries to promote wellness and recovery, staying available round-the-clock to help with all levels of care needs, and the setup aims to provide personal attention, structured days, and a supportive community, but families may want to review inspection records and visit in person before making a decision, particularly because the facility has a higher than average nurse turnover, though they can also note the above average nurse staffing hours offered to residents each day.
