    Neglectful care, understaffed, unsafe facility

I placed a family member here and removed them after 46 hours. The nurses were excellent and some staff were clearly caring, but most aides seemed indifferent, overworked, and understaffed. Response times were slow (help button ignored for long periods), patients were left alone without basic care or blankets, and medications were mishandled (missed doses and alleged morphine overuse); oxygen and emergency responses were delayed. Rooms are large but often dirty-trash on the floor, urine odor, wet linens, even black mold-and belongings went missing. I saw bedsores (stage 1-2) and heard reports of UTIs, pneumonia and a death. Communication and management were poor; despite some good people, the facility felt unsafe and neglectful. I cannot recommend it unless leadership and staffing improve.

About Fair Oaks Health & Rehabilitation

Fair Oaks Health & Rehabilitation sits over in Fairfax, Virginia, and serves people who need short-term rehab, long-term nursing care, and help with memory or dementia needs, so they've got a range of options for folks whether they're recovering from something or planning to stay longer. The place has 155 licensed beds, averages about 144 residents a day, and offers both private and semi-private rooms with features like private bathrooms, cable TV, air conditioning, kitchenette, Wi-Fi, and telephone, making it feel more comfortable for those spending any time there. This facility operates under the ownership of Hill Valley Healthcare and manages care through administrator Kathy Delimba, and there's a team with nurses, aides, nurse practitioners, case workers, physical and occupational therapists, and all the necessary supervisors and unit managers, making sure residents get medical attention day and night.

You'll find a rehab gym here, and they do inpatient rehabilitation, wound care, orthopedic rehab, post-surgical care, oncology, pulmonary, bariatric services, and tracheostomy care, alongside speech, occupational, and physical therapy programs, and there's a special PARC (Post Acute Recovery Care) unit for more complex recoveries, so there's a good bit of specialized attention for different conditions and needs. For those dealing with memory issues, there's specialized Alzheimer's and dementia care as well as programs focused on those challenges, and hospice, palliative, and respite care are all available, making it possible to meet a wide range of health concerns as people age or deal with illnesses.

They've got amenities like a bistro, restaurant-style dining with chef-prepared meals, and staff that'll handle dietary needs-so if residents have allergies or need diabetic options, they seem to keep up with that-and there's plenty to do with things like daily activities, game rooms, walking paths, a beauty salon/barber shop, music and arts programs, library, and even a movie theater, plus a patio garden and outdoor recreation area if someone wants fresh air, and community services for housekeeping, laundry, transportation, and concierge help. Spiritual support is available too. Residents get to choose from private or semi-private rooms, and there are services for both nursing home and residential care, so different needs are covered, and families can expect personalized plans of care based on comprehensive resident assessment systems in place.

Licensed as a nursing facility (NFS), Fair Oaks takes Medicaid and Medicare, is part of both the Virginia Health Care Association and the Virginia Center for Assisted Living, and is classified as a Kaiser Core facility for seamless care transitions for Kaiser Permanente members. The place has reported a lower nurse turnover rate than the state average-which can be comforting-but does show a bit less nurse time per resident compared to state averages, with 3.09 nurse hours per resident per day. There have been delays in inspections; the last standard inspection was over two years ago, well outside the usual cycle, and the most recent report flagged 43 deficiencies, including two tied to infection control, but the facility keeps working on its infection prevention practices and follows rules about food service, resident rights, and safety.

Ownership involves several holding companies, with both direct and indirect owners, so it's part of a broader network, and management is handled by a local administrator. Folks have said positive things about staff quality and the care they get, and the staff are around any time for both residents and families needing help by phone or email, so support's always available. The main idea at Fair Oaks Health & Rehabilitation is providing health and rehab services, with an approach centered on attentive, "People First" care, staying focused on recovery, comfort, and meeting every person's needs throughout their stay. You can always go to their website if you want more details.
