    Excellent activities, unsafe nursing care

I had a mixed, mostly worrying experience. Therapy and activity staff were excellent - engaging therapy, bingo, holiday parties and kind cooks - but nursing care was inconsistent and often unsafe: residents ignored, med and meal mix-ups (one allergic reaction sent someone to the hospital), missed personal care, and even staff eating leftovers in the hallway. The building is beautiful and quiet outside, but inside felt crowded, understaffed, hospital-like, and leadership was absent; some CNAs were wonderful while others were rough or unfriendly. Until management fixes staffing and safety, I cannot recommend this place.

About Bainbridge Island Health & Rehab Center

Bainbridge Island Health & Rehab Center sits at 835 Madison Ave N and works as a skilled nursing facility that's been around since 1999, offering both long-term care and short-term rehabilitation for people who need a safe and supportive place to recover or stay when regular home care isn't enough, and you'll find 58 licensed beds here, though some records mention 69, with most days seeing about 44 residents, so things never get too crowded. The staff handle case management, medication checks, discharge planning, and therapy right on site, from physical and occupational therapy to speech help, all using modern and hands-on techniques that support different needs. You'll see programs for respiratory, renal, and digestive care, plus specialized services like infusion therapy, cancer recovery, and palliative care, and they make personalized treatment plans for anyone who comes, letting people heal in ways that work best for them. Residents can enjoy well-kept rooms-some private, some shared-and spend time in landscaped outdoor areas or join in the many group activities, including social, cultural, learning, and recreational options like stay active programs and outings, which can help pass the time and bring a sense of community to each day. Meals here meet nutritional standards, though there have been a few slips over the years when it comes to food sourcing and handling, as noted in inspection reports, so it might be good to keep that in mind, just as there have been other deficiencies in infection control, dietary, and care standards, though nurse turnover here tends to be a bit lower than the state average, which can mean more familiar faces and some consistency for the residents. The facility is run by Extendicare, and while Ensign Services, Inc. and Pennant Healthcare, LLC also show up in records, operational control presently sits with Thomas De Oro since 2015. Medicare and most major insurances are accepted, and staff help people transition back home with outpatient therapy, either on-site in the gym or as follow-ups in their homes, which can make things easier after a hospital stay. Nurse staffing is a little below the state average at 3.60 nurse hours per resident daily, and the facility's been through renovations to make the environment more comfortable and healing. If you're looking for therapy, nursing care, or a place that can manage complex medical conditions in a calm setting with planned activities and outdoor space, Bainbridge Island Health & Rehab Center aims to provide those services, offering a steady routine, attentive staff, and a range of medical treatments, though it's worth reviewing recent inspection details if you want to be informed about its history.
