    Compassionate caregivers; management dangerously negligent

My experience was mixed. I found several truly compassionate, responsive caregivers and excellent therapists who gave personalized attention, kept families informed, and ran warm, celebratory activities. But management felt money-focused and inconsistent - we experienced neglect (residents left in soiled diapers, unattended in wheelchairs or halls, found on the floor after falls), delayed or missed meds, infections and hospitalizations (UTI/sepsis, extreme hyperglycemia), missing belongings, and billing/receipt problems. Night and weekend care seemed worst, with some staff rude or unprofessional. I would not trust this facility for 24-hour or long-term care unless you closely oversee care and verify staff/management changes.

    About Oak River Rehab

Oak River Rehab sits over on Franklin Street in Anderson, California, and people who know the place say it's a nursing home meant to help folks heal and recover, where skilled nurses and staff try to make daily living and care as smooth as they can, and the building has 143 certified beds so there's room for long-term folks as well as people staying just for a bit of rehab. You'll find the therapy rooms set up for physical, occupational, and speech therapies, and the staff's been trained in some more holistic recovery techniques, which means they try to help the whole person, not just one part or problem, so programs change depending on what somebody needs-short-term rehab, wound care, memory care, skilled nursing, and even respite care if a caregiver needs a break. There are both inpatient and outpatient rehab options, and people say the recovery spaces are good for quiet healing, plus the place tries to keep things homely so residents feel valued and comfortable, and there's a team that works on care plans and adjusts them with the nurses as things shift.

Now, it's got a Medicare 5-Star Rating, which is something to know, though you should also watch that the inspection reports note 19 different deficiencies, and some concerns are important to read about, such as two infection-related citations, one for pharmacy service and medication errors, another for not following rules on infection control, and one about the lack of PASARR screening for people with mental disorders or intellectual disabilities, among a few more, so folks looking here should check the inspection history to stay informed. Oak River Rehab is a for-profit facility run by Hudson River Opco LLC and is tied to the Pacs Group, and Gregory Hollingsworth has managed it since 2018, while nurse hours per resident per day run at about 4.03 with a nurse turnover rate of around 48.3%, which matters if steady staffing is important to you. There's always admission support-someone's at the front desk during office hours, and the team says they want to answer questions about admissions, insurance, and all the different services Oak River has, from therapy to skilled nursing to memory care, and they try to work on making each person's care plan personal, adjusting as folks get better or need more help, and even though you'll see some nice amenities and efforts to support comfort, checking on recent reports and staffing might be wise before deciding if it's the right fit.
