Royal Palms Post Acute sits in a large, pretty building at 630 W. Broadway in Glendale, and has been around since 2016, providing skilled nursing care for people who need extra help after a hospital stay, and with 140 certified beds, it's big enough to care for lots of people, and you'll find both nursing and rehab services under one roof. The place is connected to the Serrano Group for ownership, and Palms Licensee 3 LLC has been running things since January of 2016, and the managers say their approach is based on servant leadership, so staff try to give care in a kind and personal way as they help residents with things like stability, wellness, and comfort day to day. The nursing staff and support teams put in about 4.17 hours per resident daily, though nurse turnover sits at around 35.3%, which means there's a regular change of staff. There have been 86 deficiencies noted in inspection reports, including some about infection control, with F0880 being a specific area inspectors found and called out, and the place works on infection prevention and control to help keep people safe. Services here include physical therapy, occupational therapy, pain management, IV therapy, aggressive wound care, and diabetic care management, and there are both in-person and virtual ways for visitors to stay in touch with their loved ones. The staff aim to make people feel stable, fulfilled, and as well as can be across a wide set of programs, with a focus on careful medical attention, but like any skilled nursing facility, there are always things to keep working on.

Unsafe care, rude staff, neglect
I would not recommend this facility. Communication was poor and records were not released; staff gave inconsistent information and management was unresponsive and often rude. Care was sub-par - overcrowded rooms, overwhelming urine/feces odor, soiled linens, cold/burned food, neglected wound/post-op care - and I felt it was unsafe for recovery. I saw mistreatment, HIPAA lapses, and dismissive, disorganized staff, though a few nurses (Marla) and the social worker were genuinely caring. Positives: meds were given on time, aides could be responsive, and there was some therapy/entertainment and follow-up after discharge. Overall I experienced neglect and would urge regulatory inspection rather than recommending this place.
