    Caring staff alarming safety lapse

I appreciate the clean, affordable facility, helpful director, included meals and many truly caring, loving staff who make it feel homely. However, I witnessed a serious safety lapse - my grandmother wandered out of her small room and family wasn't notified for two hours - and I also saw some unfriendly/mean staff and signs of mistreatment. Overall it's a mixed experience: good care and atmosphere, but troubling safety and communication issues keep me from fully recommending it.

    About Scott Villa

Scott Villa sits in Hayward, California, where its single-story home has warm adobe siding, a front lawn with tropical plants, and a secure gate around the property, and you'll find it offers assisted living, memory care, respite care, home care, adult day care, and independent living options in a peaceful, home-like atmosphere. The community supports up to 35 residents under its California state license and keeps things comfortable and relaxed through furnished rooms, home-style meals made three times a day, and thoughtful touches like a library, recreation rooms, a garden, and paths for walking, with plenty of daily activities including movie nights, board games, music and pet therapy, planned creative time, exercise schedules, and special outings.

A caring staff stays on duty around the clock, helping with bathing, dressing, reminders, walking, and dining, plus medicine management, laundry, and housekeeping, and they offer 24-hour supervision, a call system, and support for non-ambulatory folks and those needing more help with transfers. The environment stays calm and safe, designed for seniors with memory care needs, with therapies for those living with Alzheimer's or other dementias. Meal plans can be made for diabetes or special diets, and there's always someone around to give a hand or a cheerful word if help's needed inside or out of the shower or to get to appointments, errands, or religious activities. Residents can visit a traveling hairdresser, relax in the spa or garden, or join activities that make everyone feel like family. Scott Villa's goal is to create a warm community where seniors can be comfortable, build friendships, and get respectful care.

    06 Jun 2025
Found five resident and five staff files complete, with medications aligned to doctors’ orders and Centrally Stored Medication records in order. Found adequate food supplies for seven days non-perishable and two days perishable, fire safety equipment fully functional with recent service, hot water at 116 degrees Fahrenheit, quarterly fire drills last conducted on 04/28/2025, and the disaster plan updated on 03/24/2025; residents’ cash resources are not handled.
    • § 9058
    12 Feb 2025
Identified an allegation that a resident’s medications were discarded in a container and that the destruction record was not completed, and noted the resident was discharged on 1/29/25.
    • § 87465(i)
    • § 87565(i)
    12 Feb 2025
Found that medications were generally administered as prescribed based on records and interviews; four residents listed earlier were no longer on the later roster, and only one had seizure medications with MARs properly filled.
    29 Jan 2025
Identified health and safety concerns after an unannounced visit, including a resident death not timely reported, missing incident reports, and failure to report suspected abuse within two hours. Also observed a non-working exit door alarm and a resident room with a hospital bed positioned sideways, plus damaged baseboard and wall molding with a protruding nail.
    05 Jun 2024
Found the allegation that staff did not allow a resident to leave or be locked in was unsubstantiated. Reviewing records and interviews showed a delayed egress door and alarms that sound when residents with dementia try to leave, and a resident with dementia had documentation stating they cannot leave unassisted.
    05 Jun 2024
Investigated allegations about a resident with dementia being restricted from leaving independently due to safety protocols like delayed egress doors and alarms, concluding insufficient evidence to determine the claim's validity. An exit interview was conducted.
    10 May 2024
Found safety and medication discrepancies on May 10, 2024: missing drawers and a drawer knob in a resident’s room, a razor left in an ensuite toilet, and a mismatch between medicines received for a resident and the amount listed.
    • § 87705(f)(1)
    • § 87506(a)
    • § 87303(a)
    10 May 2024
Investigated an allegation that a staff member yelled at a resident and discarded some of the resident's belongings in August 2023; video footage and interviews showed the staff member raising her voice and making gestures toward the resident. The case was closed.
    10 May 2024
Found unfounded that the resident's jewelry was worn by the roommate or that it was stolen, since the jewelry and the roommate were not present at the location under review.
    10 May 2024
Confirmed that a staff member yelled at a resident and threw away their personal belongings, but closed the complaint as unsubstantiated after finding evidence that the belongings were expired and rotten.
    • § 1569.269(a)(1)
    05 May 2023
Found that the allegation that staff interfered with personal phone calls was unsubstantiated. Interviews showed some residents could use the phone, others chose not to, and family instructions to limit one resident’s phone access—taken to protect that resident—contributed to the outcome.
    05 May 2023
Identified safety and security concerns, including a protruding pipe in the front yard, medications stored unlocked in the kitchen, sliding doors from resident rooms to the yard without auditory alarms, and cleaning products in the bathrooms, plus an ensuite hot water temperature of 105.4 degrees Fahrenheit. Noted fire extinguishers and smoke/CO detectors were up to date, a disaster drill was last conducted March 1, 2023, and updated documentation was provided.
    05 May 2023
Identified deficiencies in various areas of the facility during the inspection.
    • § 87309(b)
    • § 87703(a)
    • § 87309(a)
    • § 87705(j)
    01 Feb 2023
Identified the allegation that the administrator signed the resident's identification, emergency contact information, and appraisal/needs and services plan forms. A deficiency was cited for failing to submit required documentation by the due date, and the matter was discussed with the administrator at the exit interview.
    • § 87506
    01 Feb 2023
Found no preponderance of evidence to prove the alleged failure to safeguard a resident's cash resources and personal property.
    01 Feb 2023
Investigated allegations that staff did not properly safeguard a resident's cash and personal property and determined insufficient evidence to support these claims. Interviewed staff, residents, and a family member, and reviewed documents without identifying any violations.
    10 Jun 2022
Found no deficiencies after an unannounced infection-control inspection. Noted entry screening with hand sanitizer, daily symptom checks and temperature checks, and vaccination records kept for residents and staff; adequate PPE and antigen test kits were available, N95 fit testing on file, fire extinguishers charged, and hot water measured at 112.6F in one bathroom.
    10 Jun 2022
Inspection found no deficiencies at the facility. All required documents were up to date and safety measures were in place.
    26 May 2021

Also noted two staff entrances in use, kitchen staff not wearing masks, water temperature at 120 degrees, and an outdated fire extinguisher tag from 2020.
    26 May 2021
Identified deficiencies in facility including lack of hand washing supplies, improper use of entrances, and staff not following mask protocols.

    09 Dec 2020
Confirmed failure to notify responsible person of hospitalization and justified 30-day eviction.
    • § 87468.1(a)(8)
    30 Sept 2020
Found that the person referenced in the complaint was not a resident, and the complaint was unfounded and dismissed.
