    Kind staff, unsafe and poorly-run

My loved one lived here for two years. I'm grateful for several kind, knowledgeable nurses, therapists and caregivers (Perla, Rosi and others were wonderful), but overall the place is unsafe and poorly run. The building is run-down - broken beds, stained linens, furniture and no/spotty AC - with only one working shower for many residents and unsanitary conditions and infection outbreaks. Care was inconsistent: delayed responses, meds and IVs mishandled, transfers postponed, frequent understaffing, equipment shortages and terrible communication that forced me to constantly advocate. Personal items (hearing aids, dentures, clothes) went missing and management was defensive with no accountability. Despite the good staff, I would not recommend this facility without major fixes and a state inspection.

    About San Jacinto Healthcare

San Jacinto Healthcare is a medium-sized facility with a capacity of 99 beds and operates under a for-profit LLC ownership. It serves as both a short-term rehabilitation center and a provider of long-term care. The facility actively participates in the Medicare and Medicaid programs, ensuring that a broad spectrum of residents can access its services. San Jacinto Healthcare is not affiliated with a continuing care retirement community and does not offer programs specifically tailored for Alzheimer’s care.

The facility has demonstrated a strong performance in long-term care, providing consistent daily nurse staffing at an average of 3 hours and 53 minutes per resident. This commitment includes excellent continuity of care throughout the week, with weekend staffing equivalent to weekday levels. Residents receiving care at San Jacinto Healthcare benefit from attentive management; 100% of long-term care residents have maintained their ability to perform daily activities such as moving, eating, and using the bathroom independently. Additionally, all residents receive yearly influenza vaccinations, reflecting the facility’s focus on preventative health measures.

For those needing post-acute care, San Jacinto Healthcare has an average rating in short-term rehabilitation. Nearly half the short-term residents are able to return home after their stay, and the rate of falls causing major injury in this setting is exceptionally low. Emergency room visits among short-term residents are relatively infrequent, as are infections requiring hospitalization.

San Jacinto Healthcare undergoes routine health, fire, and safety inspections, as required for such facilities. The facility has a history of addressing areas identified for improvement, particularly regarding the supervision and safety of residents, food sourcing and preparation, professional quality standards, and the timely planning and execution of resident discharges. It has also made corrections in pharmaceutical services, respiratory care, infection control, and other patient care domains. When concerns have been identified, these have typically been assessed as causing low potential for harm and have affected few residents.

Overall, San Jacinto Healthcare is recognized for its attentive staffing, especially in long-term care, and the high proportion of residents maintaining independence in daily activities. The facility's ongoing focus on resident wellness, safety, and regular staff presence reflects its commitment to providing quality care to those who need both rehabilitation and extended nursing home services.
