    Mixed care, serious safety concerns

I've had mixed experiences: some nurses were kind, attentive, and helped with recovery, and parts of the facility can be respectful and clean for long-term stays. But too often I encountered poor care - infrequent baths/diaper changes, bad hygiene and smells, unhealthy/improper feeding, lack of activities/outdoor access, unreachable phones, and serious concerns about staff theft, chart falsification and suspected drug use; new management hasn't visibly fixed these. Until staffing, accountability and communication improve, I can't recommend this place.

    About Temple Park Convalescent Hospital

Temple Park Convalescent Hospital is a medium-sized skilled nursing facility located in Los Angeles with 99 beds and operates as a for-profit LLC. The hospital serves as both a short-term rehabilitation and long-term care provider, participating in Medicare and Medicaid programs to ensure coverage options for residents who qualify. While not part of a continuing care retirement community, Temple Park Convalescent Hospital supports individuals transitioning from acute hospital stays by offering post-acute care services tailored to recovery from events such as stroke, heart attack, infection, or accidental injury.

Residents benefit from an average of 3 hours and 18 minutes of nurse staffing per day, with comparable staffing maintained on weekends to ensure continuity of care. The facility emphasizes daily assistance with both medical and non-medical needs, including medication administration, mobility, and activities of daily living like eating, dressing, and personal hygiene. Efforts are also made so that residents retain self-care capabilities, with nearly 89% maintaining independence in essential activities.

Temple Park Convalescent Hospital focuses on safety and wellness through its approach to infection prevention, fall risk reduction, and emergency preparedness. Statistical data show relatively low rates of emergency room visits and falls with major injury among residents, as well as a strong record of flu vaccination coverage approaching universal reach. The hospital also works to facilitate timely and dignified transitions for residents discharged back to their homes, supporting a smooth return whenever possible.

Quality of care at the facility is routinely evaluated by the Centers for Medicare and Medicaid Services, focusing on health, fire, and safety standards. Recent inspection history has included a variety of deficiencies, each rated at a low potential for harm and affecting a few residents at a time. These issues have related to areas such as transfer and discharge preparation, residents’ rights, accommodation of individual needs and preferences, basic life support, daily living assistance, activities, pressure ulcer prevention, continence care, respiratory and dialysis services, and food service. Attention has also been paid to nutritional standards, securing resident-identifiable information, professional quality requirements, resident rooming preferences, abuse and neglect reporting, and timely notifications regarding transfers or discharges.

Temple Park Convalescent Hospital has paid fines in recent years, but continues to serve its community with the aim of providing comprehensive rehabilitation and ongoing nursing care. The environment is structured to offer reliable support to elderly and medically fragile individuals needing strong nursing attention and daily assistance, while also making efforts to create a respectful and comfortable setting unique to each resident.
