Modesto Post Acute Center runs a 99-bed facility that provides skilled nursing care around the clock, which means nurses and doctors are on hand day and night for those who need help with their health problems or recovery. Folks who stay here can get help with short-term and long-term care, so whether someone's recovering from a surgery or needs a place for ongoing support, they've got services in place. The staff includes 24-hour physicians, nurses, therapists, dietitians, and social service workers, with teams working together to create a care plan that fits each resident's health needs. There's a memory care unit offering supervised care all day and all night, plus meals, activities, and ongoing health management, which is helpful for people who need extra support with memory problems. The place also gives rehabilitation services like speech, occupational, and physical therapy to help people talk, move, and get stronger, and those services are led by skilled therapists. Modesto Post Acute Center works with a healthcare staffing platform, making it easier for them to fill shifts quickly with RNs, LPNs, CNAs, and other licensed staff, which helps them keep care available and steady, even when they need extra hands. The facility provides shift-based jobs, including PRN and per diem options, and uses tools and resources like articles, reports, and case studies to keep their staffing flexible and informed. Social services are there to assist both residents and their families during changes and with different needs, guiding folks through tough times or transitions. All in all, Modesto Post Acute Center focuses on skilled medical care, reliable staff, and a mix of therapy and support services for people who need both acute and post-acute help.

Severely understaffed, neglectful, inconsistent care
I had a horrific, inconsistent experience. The place is severely understaffed and I witnessed neglect - delayed wound care, urine-soaked diapers left for hours, missed meds, almost no scheduled doctor visits (I saw the doctor once in 10 days), and ordered physical therapy often never provided. Rooms are cramped, hot, and smelly at times; communication and administration were rude or unprofessional, belongings misplaced and an alleged $100 theft reported. On the positive side, many CNAs, some nurses and therapists were compassionate and skilled, and admissions staff could be helpful. I would not trust this facility for long-term care - maybe acceptable for short rehab only if you personally verify staffing, oversight and practices.
