    Jacob Healthcare Center provides skilled nursing care twenty-four hours a day, seven days a week, and the staff works hard to make sure residents feel safe, respected, and cared for while still keeping their dignity and independence close and always top of mind, and you'll see friendly team members who help with daily tasks, from bathing to getting dressed, so everyone can enjoy life in comfort. This center has a long tradition of helping people recover after hospital stays or deal with long-term illness with its skilled nursing, sub-acute care after major illness or injury, and even focused wound care, which includes a dedicated wound management team for healing complicated wounds. There's a team that takes care of things like medication management, IV therapy, and special therapies for people with neurological problems, like those who've had a stroke or have dementia, and they also work with folks who need help with breathing problems like asthma or COPD, using respiratory therapy and equipment to give the best chance for improvement. Rehabilitation is a big part of what goes on here, with physical therapy, occupational therapy, speech therapy, and outpatient therapy sessions, all aimed at helping residents gain back as much movement and independence as possible, and staff always look for ways to support each resident's personal goals.
