    Good care but inconsistent communication

I stayed here for rehab and had excellent wound care and aggressive, effective physical therapy - the nurses, CNAs and many staff were compassionate, professional and genuinely helpful, administration often stepped in, and the facility felt clean, welcoming and recovery-focused. Family updates, bilingual staff, 24/7 phone availability and attentive kitchen/housekeeping were big positives. That said, I also saw inconsistent staffing, poor communication (missed calls/meds, no in-room phones, unanswered requests), occasional odors/overcrowded rooms and troubling discharge/insurance handling, so you need to stay vigilant. Overall I felt well cared for by much of the team but would advise confirming med/visit/phone procedures up front.

    About Hillcrest Heights Healthcare Center

Hillcrest Heights Healthcare Center is a skilled nursing facility in San Diego that has 96 licensed beds and takes care of about 92 residents each day, so folks needing a lot of help can get round-the-clock care. People come here for short-term rehabilitation right after a hospital stay when they're not quite ready to go home, and some stay longer who are very frail and need skilled nursing or intermediate care. The building itself is modern and clean with a peaceful feeling, and it's just a few minutes from Scripps Mercy Hospital San Diego and close to the highway, which is handy. The staff keeps the place running 24 hours a day, and they try to focus on safety and comfort for everyone, using a therapy team known for doing a good job, helping people heal, and making sure everyone gets their own kind of care from professionally trained workers. The place has about 4 nurse hours per resident per day and a friendly staff, but nurse turnover runs at about 40%, roughly the same as the nurse staffing percentage, which says something about how hard the job is sometimes. The center has won awards for its rehabilitation services and has a five-star rating for quality measures, and does its best to create a home-like setting, but inspection reports have found 45 deficiencies since October 2018, including concerns with nutrition, like not always handling allergies and preferences as they should, and pharmacy services, including some medication errors and issues making sure pharmacists handle enough of the pharmaceutical services. Over the past years, inspectors found three infection-related issues too, so there's always areas that could be improved. Aspen Skilled Healthcare backs this place, with owners including Aspen Skilled Healthcare Inc, Jeffrey Bradshaw, Vern Brady, and Ryan Case, and Jeffrey Bradshaw has been running the day-to-day operations since 2018 through Asd6, LLC. Besides the healthcare center in San Diego, Hillcrest Heights is also known as Hillcrest Residential Care I & II in Anaheim Hills, which offers smaller board and care settings for up to 6 seniors per home, making things feel cozy and homey, with personalized services, three meals a day, and care that pays attention to special dietary needs. The facility allows tours, so you can see the rooms, meet the staff and residents, and get a real sense of how things feel before making a decision. Although Hillcrest Heights has amenities and healthcare services to try making everyone comfortable and safe, there have been some problems and reports show that while much is done right, families will want to ask questions and look around when figuring out if it's the right fit for their loved one.
