The Pavilion at Ocean Point is a full-service skilled nursing and rehabilitation facility in San Diego, California, with 133 certified beds and an average of 124 residents each day, and it's managed by Shlomo Rechnitz since 2006, with ownership shared among Pacific Healthcare Holdings, David Blonder, Katz Healthcare Investment Partnership, and Sol Majer, and also has ties to Brius Management. The place provides a wide range of care, including assisted living, independent living, memory care for Alzheimer's, continuing care, nursing homes, respite care, home care, and care homes, so there's support no matter what level of help is needed. Some specialty services are there, like orthopedic and neuro rehabilitation, dialysis care, wound and skin care, IV therapies, and a special memory care unit for people struggling with Alzheimer's, plus physical, occupational, and speech therapies, and a team of professional nurses and assistants work 24 hours every day, admitting new residents around the clock, and all are closely involved with each resident's physician. The Pavilion encourages family involvement in care meetings, and offers lots of social options-there are things like arts and crafts, movies, outdoor activities, and a host of programs just for residents, so folks can socialize, keep busy, and enjoy themselves. Meals are handled by a dietitian and everyone gets three balanced meals a day, and there are extra services on site, including dental and podiatry visits, psychiatric care, orthopedic specialists, social workers to help with planning and placement, and in-house doctors available on call. Now, it's important to know the facility has had some issues listed in inspection reports, because over the years they've gotten 90 deficiencies, with 5 related to infection control, and have been cited for not always honoring residents' rights to refuse or request care, or keeping them fully informed about health issues, and also for lacking proper infection prevention programs, which is something folks should think about. Nurse staffing hours are 4.01 per resident per day, a bit below the state average of 4.5, and nurse turnover is high at 70.1 percent compared to the state's 40 percent, yet the staff focuses on helping each person reach their best possible recovery, aiming for a respectful and therapeutic environment, and everyone tries to make sure the place feels warm and comfortable for those who live there, which matters to many. They have a map available to help new residents and visitors get used to things, and tours are open for prospective residents to come meet staff and see the facility themselves, so families can get a good look before making a choice.

Excellent therapy, serious safety concerns
I had a very mixed experience. The building is clean, housekeeping is excellent, therapy (OT/PT) and activities are standout, and many nurses and CNAs were warm, attentive, and helped with smooth admissions/discharges and rehab. However I witnessed or heard about serious problems - medication errors and missed/ignored call lights, inconsistent hygiene, resident neglect/abuse, faulty equipment, theft/security concerns, and management covering up issues with high staff turnover. In short: great therapy, friendly staff, and a pleasant environment at times, but safety, medication and supervision problems make me very wary.
