    Excellent therapy, serious safety concerns

    I had a very mixed experience. The building is clean, housekeeping is excellent, therapy (OT/PT) and activities are standout, and many nurses and CNAs were warm, attentive, and helped with smooth admissions/discharges and rehab. However I witnessed or heard about serious problems - medication errors and missed/ignored call lights, inconsistent hygiene, resident neglect/abuse, faulty equipment, theft/security concerns, and management covering up issues with high staff turnover. In short: great therapy, friendly staff, and a pleasant environment at times, but safety, medication and supervision problems make me very wary.

    The Pavilion at Ocean Point is a full-service skilled nursing and rehabilitation facility in San Diego, California, with 133 certified beds and an average of 124 residents each day, and it's managed by Shlomo Rechnitz since 2006, with ownership shared among Pacific Healthcare Holdings, David Blonder, Katz Healthcare Investment Partnership, and Sol Majer, and also has ties to Brius Management. The place provides a wide range of care, including assisted living, independent living, memory care for Alzheimer's, continuing care, nursing homes, respite care, home care, and care homes, so there's support no matter what level of help is needed. Some specialty services are there, like orthopedic and neuro rehabilitation, dialysis care, wound and skin care, IV therapies, and a special memory care unit for people struggling with Alzheimer's, plus physical, occupational, and speech therapies, and a team of professional nurses and assistants work 24 hours every day, admitting new residents around the clock, and all are closely involved with each resident's physician.
