About Gem Oaks Assisted Living RCFE

Gem Oaks Assisted Living RCFE sits in Thousand Oaks, CA, and the place has a real home-like feel since it's a single-family home with about 3,500 square feet on a third of an acre in the Lynn Ranch Community, so you've got open space, a landscaped backyard, a patio, and views of the Conejo Valley Hills which make for nice, peaceful afternoons. The house has four private bedrooms, one shared bedroom, two full bathrooms, and a powder room, all accessible with wheelchairs, and the dining and kitchen spaces get lots of natural light, which makes things feel airy and open. Residents have a 65-inch TV with reclining couches in the common room and each bedroom has a 4K TV, so everyone can watch what they like, and the cove ceilings and two fireplaces in the common spaces help it feel comfortable. You'll find the live-in administrator and a well-trained care team, and with a better than 3:1 caregiver-to-resident ratio, staff get to know the residents and take care of them like family members, making life a bit easier for people who need help every day. Everyone gets home-cooked meals and snacks each day, with meal plans that work with diet restrictions if needed, and the staff help out with things like bathing, dressing, using the bathroom, walking, and managing medications, making sure everything's safe and as easy as possible. There's 24-hour support, and the care goes from simple help for people who want to keep their independence all the way through to help for those who need total support, whether because of mobility issues, Alzheimer's, or dementia-in fact, the staff run memory-enhancing activities and programs for memory care, making sure people are supervised and supported all the time if they need it. Therapy services are available to support health and well-being, and hospice waivers are approved if needed. There are private and shared suites, along with plenty of daily housekeeping-linens, clothes, bedrooms, bathrooms, and all the open spaces are kept up. Family gatherings, barbecues, birthday parties, senior-friendly games, and events pop up regularly to help people stay social, and activities focus on hobbies, learning, and something new to do so people don't feel left out or bored. Transportation for medical appointments gets coordinated, and there's help with family visits, so everyone can stay connected. With regular oversight, an on-site administrator, and a staff dedicated to treating residents with dignity and respect, Gem Oaks offers assisted living, memory care, skilled nursing, respite care, and supports a broad range of personal and medical needs, all under a license issued by the California Department of Social Services, number 565850203, currently good through August 18, 2025. The facility has an A+ BBB rating, though it isn't accredited, and counts as a continuing care retirement community where seniors can stay and get support as care needs change.
