Harbor Post-Acute Care Center is a healthcare facility that offers post-acute care and has 127 certified beds, usually caring for around 112 residents each day, and you'll find most of the staff speak English, though some can help in other languages too, and what you mainly get here is a mix of skilled nursing, long-term care, hospice care, and rehabilitation services like physical medicine. The nurse staffing comes out to about 3.90 hours per resident per day, which is less than the state average of 4.5, but their nurse turnover rate is lower than most, about 25.8% compared to the state average of 40%, which can mean the nurses around here tend to stay a bit longer with their residents. Harbor Post-Acute Care Center has had a total of 62 deficiencies in inspection reports, with 8 of those related to infection control, showing that there have been issues in the past with keeping things up to the infection standards, and there have also been citations for not always allowing residents to return after a hospital or therapeutic leave, along with cases when the call system in bathrooms and bathing areas didn't work like it should. The facility's been affiliated with Charis Trust since December 22, 2016, and the staff aim to make the place supportive for both residents and their families, while keeping the environment as comfortable as they can, so if you need care after a hospital stay, rehabilitation, ongoing nursing attention, or hospice services, this is the kind of thing offered at Harbor Post-Acute Care Center.

Inconsistent care; attentive staff, problems
I had a mixed, mostly stressful stay. I found some caregivers loving, attentive and physical therapy excellent, but too many staff were uncaring, rude or clearly overworked and understaffed. Cleanliness and equipment were inconsistent-my room was okay but restrooms, beds and potty chairs were dirty or broken-food and pain management were often poor, and I was alarmed the doctor didn't review records and by reports of a COVID outbreak and neglect. Because of the inconsistency I can't fully recommend this place.
