    Good rehab but stay vigilant

I placed my father here and overall I'm grateful: the nurses and therapists were amazing, rehab actually helped him walk again, the food and kitchen staff were lovely, and the facility has been beautifully updated under new ownership. Communication and family involvement were generally clear and medical care felt strong and responsive. That said, there were worrying lapses-early neglect (bedsores that weren't addressed until I complained), staffing shortages, spotty communication (delayed dialysis calls, nonfunctional call buttons), and isolated theft reports-so you need to advocate for your loved one. Great rehab and caring staff, but stay vigilant.

    About Vacaville Ranch Post Acute

Orchard Post Acute Care is a medium-sized nursing facility with 87 beds, operating under for-profit, corporate ownership. The center participates in both Medicare and Medicaid programs, making it accessible for a range of patients in need of short-term rehabilitation or long-term care. Orchard Post Acute Care is not part of a continuing care retirement community and does not offer connected Alzheimer’s or memory care units within a hospital setting.

The facility holds an overall average rating for both its short-term rehabilitation and long-term care services. Residents who arrive for short-term rehabilitation often do so following a hospital stay for conditions such as stroke, heart attack, infections, or physical injuries. About 54.2% of short-term residents were able to return home after their stay, demonstrating a moderate success rate in rehabilitation outcomes. Nurse staffing levels are maintained at approximately 5 hours and 30 minutes per resident each day, aiming to provide consistent care and attention. However, staffing levels on weekends may not always match weekday coverage, which has been noted as an area for improvement.

During recent inspection cycles, Orchard Post Acute Care underwent a thorough examination of its health, safety, and infection control practices. Inspectors reviewed a variety of aspects within the facility, including the maintenance of a safe, clean, and homelike environment, ensuring accident prevention and providing appropriate pain management for residents who require such services. They also assessed the establishment of plans to meet each resident’s immediate needs upon admission, the honoring of personal preferences and values, and the giving of proper notices regarding Medicare and Medicaid liabilities.

Preventative health measures such as the annual influenza vaccination are promoted within Orchard Post Acute Care, with 97.4% of residents receiving the flu vaccine according to the latest available data. The staff also monitors the ability of long-term care residents to self-care, with around 71% maintaining independence in activities such as moving, eating, and using the bathroom. Emergency room visits from both short- and long-term residents are reviewed regularly to track the facility’s response to acute health needs.

Infection prevention and control programs, as well as antibiotic monitoring, remain core focuses at Orchard Post Acute Care. The most recent inspections found the need for further attention in these areas, particularly in the documentation and education regarding COVID-19 vaccination for both residents and staff. Additional areas of focus included the regular submission of complete and accurate nurse staffing information, proper storage and handling of resident food, safeguarding of medical records, and monthly reviews of residents' drug regimens by a licensed pharmacist to avoid unnecessary medications.

Orchard Post Acute Care has experienced a number of citations and fines in the recent past, which have prompted corrective actions and heightened emphasis on resident safety and regulatory standards. Every inspection cycle brings renewed attention to areas such as maintaining resident records according to professional standards, providing safe and adequate supervision to prevent accidents, meeting all documentation requirements, and implementing programs that ensure the wellbeing and dignity of every resident. Through ongoing evaluations, the facility continues to address these areas with the goal of providing a safe, comfortable, and supportive environment for all who reside there.
