The center cares for up to 79 people and is run by Volunteers of America, and offers many care options, including Memory Care with special Alzheimer's programs, Assisted Living, Independent Living, Home Care, Adult Day Services, Skilled Nursing and Long-Term Care, Respite Care for short stays, Hospice Care, and Home Health Care that's Medicare-certified, with teams of licensed nurses, rehab therapists, nursing assistants, and social workers who develop personalized care plans with residents and physicians.
Horizons Care Center provides help with daily needs like bathing, dressing, grooming, using the toilet, meal preparation, laundry, housekeeping, and also group or home-delivered meals, and if needed, two-person transfers for extra help, plus wound care, injections, and catheter care.
