    Compassionate staff, but severe neglect

    I had a deeply mixed experience. Parts of the facility felt clean, home-like, and staffed by warm, experienced people who provided compassionate care and good therapy, but I also witnessed or was told about serious problems - chronic understaffing, long call-light delays, poor/insufficient and often cold food, dirty hallways/rooms, laundry problems and reports of theft or falsified charting. Most concerning were instances of neglect (residents not helped to eat or use the bathroom, dehydration, infection, catheter issues and hospital transfers). I appreciate the caring employees I met, but I can't fully recommend this place without major staffing, food, and management improvements and closer oversight.

    Madison Health & Rehabilitation Center is a single-story skilled nursing and rehabilitation facility that offers a mix of health and rehabilitation care for seniors, so folks can get help with a range of needs, whether it's long-term nursing, short-term recovery, or daily living assistance, and the staff provides specialized nursing care between 12 and 16 hours a day with a 24-hour call system, always making sure there's a licensed nurse on site day and night, and you've got help with things like bathing, dressing, getting around, and medication management available for those who need extra support. The facility takes pride in its variety of amenities, with furnished rooms that come with private bathrooms, Wi-Fi, cable TV, air conditioning, and kitchenettes, and if you want to keep busy, there's a library, arts and activity rooms, a game room, a fitness room, outdoor gardens, walking paths, and even a spa and sauna, not to mention social activities like movie nights and group programs to help folks get to know each other, plus they allow pets if you want to bring one along for company. The rehabilitation team uses state-of-the-art equipment and focuses on quality clinical care, covering everything from wound care and physician visits to restorative rehabilitation, dietary counseling, dental and vision care, and disease management, and family and resident councils meet to share concerns and work on improving care, with input from families considered in developing individualized care plans. The place puts an effort into hospital readmission reduction and has disease management and early intervention plans, and takes Medicare and Medicaid as payment options, plus provides transportation for medical appointments or outings, housekeeping, and laundry services to make daily life easier. The management has changed a few times over the years, and current managers include James Jarman, Moshe Kelman, and Myra Raymer, with ownership split among Benjamin Landa, Goldie Platschek, Alexander Platschek, and David Rubenstein. There are usually about 82 residents a day, and with 92 certified beds available, nurse turnover is about 49.4%, and each resident can expect about 4.43 nurse hours per day. Inspection reports show a focus on nursing home compliance, and there have been deficiencies cited-16 in total-including two that have to do with infection control and others about care planning and helping residents keep their daily living abilities, so these are things folks should consider when looking at the center, but they do develop care plans after thorough medical assessments with input from both residents and family, and try to ensure services get to everyone regardless of their background or payment source.
