Bay Shores Senior Care & Rehab Center sits on Midland Road in Bay City, Michigan, and it's been around for more than 30 years, run by Nexcare Health Systems since 2004, so people tend to know it in the area, and everyday there's usually about 113 people living there, with both young and older folks sharing the community. The center has 126 certified beds and gives care for lots of needs, like skilled nursing, long-term and short-term rehabilitation with physical, occupational, and speech therapy every day of the week, along with respite care if someone needs a break, plus memory care for people with Alzheimer's or dementia, and even palliative care when that's needed. Folks can stay in private suites, watch their own flat-screen TV, use the Wi-Fi, and eat meals in a restaurant-style dining room, with snacks and activities offered daily, including outings now and then, and there's also a salon and barbershop. The team includes skilled nurses and aides who manage medications and provide 24-hour RN coverage, and an interdisciplinary team helps each person work towards particular goals, whether they're bouncing back from an illness or settling in for long-term care, and the staff gives spiritual support too. The building tries to keep things clean and safe with tools like the Clorox Total 360 System, Aquawing Ozone Laundry, UV lights, plexiglass shields, and regular staff health screenings like temperature checks and weekly COVID-19 tests, and they've all had extra training for these safety steps. Still, reports have found some problems over the years, including infection control issues and a higher nurse turnover than average, and inspectors have written up 32 deficiencies, four about infections, though none caused actual harm, just a risk for it, and there were also notes about care not always matching what the doctor ordered or what residents wanted-again without any actual harm happening. Bay Shores is accredited, does hold a 5-star CMS rating and the AHCA Bronze Award, and works with programs like the Certified Assisted Living Director, and residents can expect rehab and outpatient therapies, medication help, and individualized plans for treatment, though nurse hours per resident are a bit lower than the state average. Overall, Bay Shores Senior Care & Rehab Center offers a busy, family-style setting with options for both short and long stays, aiming to fit different needs, always in a place that tries to be both welcoming and safe for everyone who comes through the doors.

Kind staff, overall unsafe care
I'm torn: the building, therapy team, and several aides were kind and skilled, but overall care was inconsistent and often unsafe. I experienced long call-button delays, understaffing, ignored wounds and catheter/UTI issues, poor feeding/hydration, and delayed medical attention that led to serious infections and hospital transfers. Management and some staff were dismissive or rude, though a few caregivers showed real dignity. I would be very cautious about sending a loved one here.
