    Improved care, but safety concerns

I've had a mixed experience: under new ownership the place has clearly improved - caring, long-tenured and attentive staff, good OT/PT, helpful social work and financial teams, clean/attractive rooms, decent food and lots of activities. However I've also seen and heard serious ongoing issues: short-staffing, medication delays, inconsistent communication, theft/safety concerns and an aging building with maintenance and infection-control lapses. I'd say it's much better than its prior reputation but check staffing, meds and valuables closely before deciding.

About Seacrest Rehabilitation and Nursing Center

Seacrest Rehabilitation and Nursing Center sits in Monroe, MI and offers a wide range of care, so people can find almost anything they need-active adult living, independent and assisted living, skilled nursing, memory care for folks with Alzheimer's or other dementia, home care, Medicare-certified home health, adult day services, hospice support near the end of life, and respite care if families need a break. The building's been renovated, which folks say makes it feel more comfortable and healing inside, and they've got special units for long-term care, transitional care, a secured neighborhood for memory care, and lots of rehabilitation and therapy programs for those who need to recover after being in the hospital. The place runs 24 hours a day with skilled professionals on hand and has a nurse staffing level of 3.22 nurse hours per resident per day, plus a nurse turnover of 23.7% in one year, with about 85 folks living there on average even though they can fit 131 residents and up to 150 beds for rehabilitation use. They've got dedicated staff who treat folks and their families kindly, making sure to keep in touch and create relationships, which people notice about the place. Staff get training for dementia care, and the rehab team develops personal care plans that bring in the resident, their folks, and the doctors, so everyone stays on the same page, and they even have specialties like Orthopedic Rehabilitation, Pulmonary Care, Cardiac Telemetry, and an in-house dialysis center. Seacrest accepts Medicaid and Medicare, and they're known for above-average inspection and overall ratings, although their quality measures are just a bit below average and staffing is about average. Their skilled services run the whole range, including medication help for those in assisted living, respite and hospice care, long-term care, and sub-acute rehab that aims to cut down on hospital stays and keep folks recovering well at home. They've had reports of compliance and deficiencies from surveys, including issues with food handling, environment safety and cleanliness, and making sure residents keep their mobility as best they can, which are reflected in citations for nutrition, environmental, and quality of life and care. They've reported 18 total deficiencies, including two for infection control. Ownership falls to Renewal Healthcare LLC, with Eliyahu Freund, Moshe Gottlieb, and Joshua Koenig having stakes. This place stays for-profit, but is widely seen as having good health inspections and a compassionate approach, focusing on personal care and making the best of each resident's quality of life, with a bunch of innovative activities and personalized rehab plans that make people feel involved and respected, even though the facility does have some things they're still working to improve as shown in inspection reports.
