San Juan Center sits at 806 W Maple St and stands as a nursing home with a 4.3 rating from 21 reviews, offering all kinds of care for seniors, those needing rehabilitation, or anyone with long-term healthcare needs, and it's run under the guidance of administrator Joshua Martin, now there's a kind of steadiness about it because they keep the doors open 24/7, so someone always finds support day or night, and the staff works with nursing, dietary, social services, and activity teams to make sure all sorts of needs get met, so every person has a plan shaped for them and their family, which you don't see everywhere, and they call their short-term programs ShortStay, while their longer arrangements fall under LongTerm care, covering things like memory care for those with Alzheimer's or dementia, and then they offer post-acute skilled nursing, hospice, palliative, and supportive care. At this facility you'll find physical, speech, and occupational therapy, all supervised by folks using state-of-the-art equipment and certified wound care nurses manage any wound care needs, and rehab can be for short-term or long-term plans, sometimes even just outpatient rehabilitation if that's what a doctor suggests, and that's on top of the classic nursing home care. Somebody might need help with daily things like dressing or bathing, so the assisted living communities on site will handle that, and then there are services for people wanting independent living or a senior apartment too, and housekeeping, laundry, transportation, and meal programs-one to two prepared meals every day-mean people can relax and count on the basics being looked after, while special resources like geriatric care management, legal help, and medical equipment are part of the package if someone needs them. Education and training come from their webinars, workshops, and even video training, which helps family members keep up and understand the best ways to help, and when it's time to figure out long-term care insurance or caregiving options, San Juan Center steps in with guidance and answers. Each private room has a TV and a phone to keep in touch and feel at home, and activities-some for the body, some for the mind-are matched to each person, so everyone has a chance to stay sharp or just enjoy themselves. The New Mexico Health Care Association counts San Juan Center among its members and the whole operation is built to keep every patient steady on their feet or recover as best as they can, with personalized rehabilitation plans and attention given to any transition so no one feels lost in the move from home or hospital. At San Juan Center, everything's designed to help seniors and people of any age who need it to keep their independence or recover with as much comfort as possible, always with licensed social workers to stand by during all those changes life throws your way.

Great care, but expect delays
I had a loved one at this facility and overall the therapy team, nurses and CNAs were amazing - friendly, compassionate, hardworking, and the PT/OT got great results; the place felt clean, welcoming, and activity-rich. That said the building is old and sometimes crowded, administrative/transfer and insurance issues caused delays, and response times varied - I saw missed checks, delayed help and occasional inattentive staff that need fixing. Management is caring and reactive, but I'd recommend with reservations: excellent staff and care overall, just be prepared to stay involved and push for improvements.
