The Hurlbut sits about 2.6 miles outside of Henrietta, New York, and it's been around since 1969 when Robert H. Hurlbut started it in honor of his parents, with his children now representing the family's third generation of care. This place has 160 certified beds and cares for about 113 residents each day. The Hurlbut is part of the larger Hurlbut Care group, which is family owned and operates 11 locations, so staff often draw from over 50 years of experience. The nursing home is managed by Mark Curletta since July 2021 and it's owned by Robert Hurlbut. It runs as a for-profit business. Residents get 24-hour skilled nursing care, and you'll see staff trained in geriatric care, some who've seen a nurse turnover rate of 46.3%, with an average of 3.80 nurse hours per resident per day. The facility provides post-acute nursing and rehabilitation services every day, including specialized therapies like physical, occupational, and speech, and offers care for people needing wound care, IV therapy, pain management, or help with tubes and feeding. Short-term rehab and long-term care are available here, and there are agreements with area hospitals for extra support. Staff also include social workers and a dedicated care team of over 1,400 people across the organization. Amenities include private and semi-private rooms, nurse-call systems in bedrooms and bathrooms, cable TV and telephone in rooms, beauty and barber shop services, and full housekeeping and laundry. Activities aim to fill the days with things like sing-alongs, picnics in enclosed courtyards, outings to restaurants or concerts, pet therapy, art therapy, and religious services with local clergy. The space is wheelchair-accessible, and outings use a wheelchair-accessible bus. The Hurlbut holds an overall 2-star rating from the Centers for Medicare & Medicaid Services and holds an average rating of 7.2 out of 10, placing it 37th in the city. Standard inspections have found 13 deficiencies, with three of them related to infection control, and past violations recorded. There's a quality assessment and assurance group that meets at least every quarter to check on care, which includes making sure policies prevent abuse, neglect, and theft, and that the place keeps up with federal, state, and local rules. Residents and families can visit for scheduled tours to meet the staff, see the rooms, and learn about daily life.

Compassionate staff, but unsafe facility
I saw genuinely caring CNAs, nurses and an excellent rehab team, and the rooms are pleasant with lots of light, TVs and daily activities like bingo and PT/OT. But the place is inconsistently maintained-dirty hallways, pests, broken furniture-and severely understaffed and mismanaged, with staff sometimes arguing or covering for each other. I experienced poor medication communication and dosing/record-keeping errors, inconsistent meals (the dietician was helpful but special-diet menus were missing), and high out-of-pocket costs for what felt like poor value. Despite some dedicated, compassionate staff, the safety, cleanliness and leadership problems mean I cannot recommend this facility.
