Five Oaks Rehabilitation and Care Center sits at 413 Winecoff School Rd in Concord, NC, though some folks in Kannapolis know it too, and what you see when you come in is a home-like setting with 160 skilled nursing beds, a sunroom where light comes through nicely, and patient rooms that seem designed to help people both rest and recover without much fuss. The place runs all day and night, and if you take a look around, you'll spot a rehab gym with equipment meant for different kinds of therapy, which helps both long-term residents and folks just staying for a little while, especially when someone's coming out of the hospital and needs to get back home. People use these spaces to work one-on-one with therapists aiming to help each person get back their best level of comfort and movement, and there are both community spaces and private suites for different needs, so there's a spot for group activities or quiet relaxation. The staff are licensed and include nurses and therapists, with an administrator, Jonathan Thomas, handling things, and the team brings care services, rehabilitation services, and support with managing pain and daily routines, although recent state reports show nurse hours per resident each day at 3.71, which is just under the North Carolina average of 3.8, and there's a nurse turnover rate of 62%, which is a bit high compared to some other places nearby. Five Oaks carries the title of Skilled Nursing Facility and Physical Medicine and Rehabilitation Clinic, and it's managed by Venza Care Management, so there's a structure to things, though inspection reports show 30 deficiencies, one tied to infection, plus a few citations for infection control, supervision, accident hazards, and pain management. Some staff speak English, and a few know other languages too, so there's help with communication when needed. While the reviews average out at 4.2 from about 125 people, and there's a contact form for those wanting info or a tour, the facility's not taking new patients at this time unless you check in about openings, since beds fill up quickly, and care options do change from time to time depending on what's available.

Compassionate staff but dangerous neglect
I'm grateful to some compassionate therapists and CNAs who helped my relative recover - rehab was effective and parts of the building were clean and home-like. But chronic understaffing, slow call-bell responses, missed bathroom assistance, incontinence neglect, dehydration, bed sores, infections (including COVID and catheter-related), hospitalizations and even deaths left me alarmed. Communication and maintenance were poor, meals often bad, and the place felt run-down in spots; staff clearly tried but were overworked. I strongly warn others - do not send your loved one here unless things change.
