Autumn Care of Fayetteville sits on five peaceful acres in Fayetteville, North Carolina, surrounded by mature trees and a secure courtyard you can see from most rooms, which makes things feel calm and homey, and with 90 skilled nursing facility beds, this place offers skilled nursing care all day and night for those who need support, and you'll find both private and semi-private rooms for long-term residents, so folks have some choice about how they want to live. There's a real focus on respectful and compassionate care, and a team of dedicated professionals like nurses, aides, and an administrator named Barbara Collins work together with doctors to make sure each person gets a plan that fits their own needs, whether that means rehabilitation after a hospital stay, help with daily living for folks who want to stay as independent as they can, or memory care for residents dealing with dementia or Alzheimer's, where the staff really pay attention to safety, comfort, and meaningful activities to help with both physical and emotional needs. For rehabilitation, there are modern therapy gyms with equipment such as the Omnicycle, Virtual Rehab Systems, E-stim, ultrasound, and Vital Stim to support both short-term rehab and longer recovery plans, and the place even helps with paperwork to make moving from hospital to rehab and then home a bit smoother, and will talk you through things like pre-surgical exercise or getting used to new devices before a planned hospital stay, as part of what they call the PATH program. Residents can expect meals served restaurant-style, laundry services, and help with transportation for things like dialysis, and in addition to medical care, Autumn Care of Fayetteville offers life enrichment activities that might include fishing trips, spa days, community birthday parties, and holiday events, which helps keep things lively and social, plus there are safe outdoor spaces for walking or just enjoying the view, especially for those in the memory care unit. There are programs for both assisted living and personal care, which put an emphasis on keeping residents as active and independent as possible, and respite care options let families get a break or decide if this kind of care is the right fit. This facility also complies with federal civil rights laws, welcoming people regardless of background, and is recognized by the American Health Care Association's 2020 Quality Initiative for its focus on meeting quality care goals. The staff takes time to build individualized care plans for every resident, whether someone's staying for a few weeks or long-term, and for those looking ahead to a planned surgery or hospital stay, pre-registration helps make the move into rehab easier and less stressful. You'll find cable TV, internet, hospice care if needed, social services, and even salon and barber services available, so residents can keep up with everyday things in a familiar environment. The approach here is steady and team-based, always working with families and physicians to support both health and daily living, and the idea throughout the whole place is to provide support while helping folks keep as much independence and dignity as possible.

Inconsistent care, neglect, safety risks
I had a mixed but mostly negative experience. A few nurses and therapists were compassionate and competent, but too many staff were unresponsive, rude, or lazy - long call-button waits, unanswered phones, and poor communication from administration. I saw neglect: soiled clothing/bedpans left, hygiene skipped, bedsores, rushed or ill-prepared discharges, and inconsistent therapy/medication handling. Cleanliness and care swung wildly between excellent and unacceptable; I cannot recommend this facility unless you want to gamble with your loved one's safety.
