    Good rehab but dangerous lapses

I had a mixed experience. The rehab team (PT/OT), nurses and many aides were excellent-compassionate, helpful, and made my sister feel welcome; Sam in social work handled admissions/discharge smoothly. The facility was clean, had engaging activities, and daily housekeeping. But staffing and management problems caused dangerous lapses: dehydration leading to hospital stays, long unanswered calls/screaming, wet diapers left for hours, poor diabetic/menu attention, and spotty communication about transfers. Some staff and agency CNAs were rude; QA/administration issues undermined care. Good for rehab/short stays if you monitor closely, but be cautious.

About Louisburg Healthcare & Rehabilitation Center

Louisburg Healthcare & Rehabilitation Center, also known as Liberty Commons or Franklin County, sits at 202 Smoketree Way in a quiet residential neighborhood in Louisburg, North Carolina, and has 92 beds for residents needing skilled nursing or intermediate care, and folks will often see those very frail who depend on daily nurse support, but also people staying short-term after surgery or illness to get rehab before heading home if they're able. Syreeta Latta, the administrator, oversees care that includes 24-hour skilled nursing teams like registered nurses, licensed practical nurses, and certified nursing assistants who keep close watch on health, giving medicine, helping with chronic diseases, wound care, IV therapy, tracheostomy support, and keeping up mobility, with full-time physicians on site and special help from wound care doctors, a physiatrist, geriatric psychiatry, and even podiatry. The place has a dedicated rehab unit, outpatient therapy, private and semi-private suites, and an Activities of Daily Living Suite in the rehab gym, plus that's where folks get help with physical, occupational, and speech therapy up to seven days a week, and they do things like balance, joint movement, daily tasks, swallowing, and communication, and sometimes things like diathermy for pain or vital stimulation for swallowing. Residents can feel some comfort from roomy layouts with natural light, wheelchair-friendly design, in-room call systems for help, and chances to personalize their rooms, while the staff stays alert to safety, fall prevention, and emergency needs at all hours. Residents get chef-prepared meals with diet changes if needed, and there's always help for medication, bathing, dressing, or getting around, and if folks need a break, respite care is available with full care for a few days or weeks. The place also covers hospice and palliative care, focusing on pain management and emotional support for families, with private room options and a calm, respectful approach for end-of-life wishes. There's a sense of community with chances for companionship, life enrichment activities, social gatherings, and outdoor seating in landscaped courtyards or indoor spaces, and daily life feels easier with housekeeping, laundry, linen service, on-site salon and barber, and transportation coordination for off-site appointments or doctor visits. Some rooms come with flat-screen TVs, free cable, WiFi, and in-room dining, and everyone has access to amenities either private or shared, with medical plans that cover secondary conditions, daily wellness checks, and regular adjustments to therapy progress. The care team works with each resident and their family to set up care plans that fit their needs, with the goal to regain skills and help avoid going back to the hospital. The facility admits patients seven days a week and has programs and services set up for people needing long-term living in a familiar, steady, and safe environment.
