    Unsafe neglectful poorly managed facility

I stayed here and overall it was unsafe and disappointing. Staff were often unresponsive or overworked - meds were delayed or denied, doctor visits and PT were missed, meals skipped, transportation unreliable, and complaints sometimes met with retaliation. The building is dated and poorly maintained (peeling paint, odors, roaches, boxes at the entrance), housekeeping and food are terrible, and I witnessed neglect (soiled diapers left, missed turns, bedsores/falls, wrong meds). A few people were kind and helpful - Frances, Amanda and David and some nurses/recreation staff - but inconsistent care, poor management, and lack of transparency make this not worth the cost; I will not be returning.

    About Siler City Center

Siler City Center sits as a skilled nursing facility with 150 beds, operated by administrator John Alvarez and part of SunBridge Healthcare Corporation, giving folks a place to recover after illness or injury, or to settle in for long-term care, and they've also earned the AHCA Bronze Quality Award for their commitment. You'll find the rooms-both private and semi-private-have tubs, wheelchair accessible showers, climate control, and internet access, and they offer special Rehab Recovery Suites for recovery. Folks get home-cooked meals with vegetarian options, laundry and housekeeping, in-room dining, and mail delivered, and the dining room and indoor and outdoor common areas give people spaces to be together, along with an activity room, gardens, and courtyards. There's a beautician on site, a barber shop, and plenty of community spaces, so folks staying a short while or for longer have what they need for comfort and daily routines.

The Center welcomes people in need of skilled nursing for many medical needs, like cardiac issues, pain management, wound care, stroke rehabilitation, diabetes management, and even complex care like memory care for Alzheimer's and dementia, with around-the-clock nursing, personal care, case management, and medication help, plus help for folks needing incontinence support, colostomy care, IVs, and even palliative and hospice care when the time comes. For anyone needing rehab, there are physical, occupational, speech, respiratory, and restorative therapies available, including outpatient treatments and special help for things like joint replacement, amputation, and orthopedic injuries. The staff brings experience and attention, earning recognition for their dedication and even using technology, like helping families connect with residents on Zoom calls.

The facility stays busy with recreational, cultural, social, religious, and educational activities, keeping things lively with outings, field trips, pet therapy, and community events to help residents and families stay engaged; there are also family and resident councils for sharing ideas and concerns. They provide transportation and parking for residents, plus rides to things like off-site dialysis. They manage dietary, nutritional, and pain needs, and coordinate services like x-rays, dental, podiatry, vision care, and pharmacy deliveries, with specialists including attending physicians, nurses, physiatry, and contract rehab teams who help set up treatment or discharge plans.

Folks who come to Siler City Center will notice staff that aim for a supportive and friendly atmosphere, with a smoke-free environment, an emergency alert system, and security alarms. As a Medicaid and Medicare provider, and accepting most private insurances, they're part of the North Carolina Health Care Facilities Association and also seen in the National Alliance for Care at Home, so there's help for different income levels and medical needs, whether it's for short-term stays, respite visits, or something longer. The place is designed to make things as comfortable and manageable as possible, focusing on what people need without making a fuss about it, just steady care and a clean, welcoming place to be.
