The facility offers skilled nursing care and has 106 certified beds, currently averaging about 101 residents each day. Residents can find respite care if their family caregivers need a break, or receive short-term rehabilitation after an illness or injury. The team provides memory care for those with Alzheimer's or other types of dementia, and they offer both palliative and hospice care for residents needing extra comfort or end-of-life support. Therapy services include physical, occupational, and speech therapy. Swannanoa Valley's nurse staffing sits at 3.23 hours per resident per day, which is below the North Carolina state average, and the nurse turnover rate is high, standing at 69.3%. The facility has drawn several inspection deficiencies recently-33 total-including issues around infection control, pest management, maintaining a safe and clean environment, and failing to ensure a hazard-free home for residents. Some inspections have also noted failures to protect residents' rights and prevent abuse or neglect. Despite these concerns, many in the local community recognize the facility for its focus on compassionate care and its aim to treat residents like part of an extended family. The staff tries to provide personalized support, involving families and aiming for a home-like setting with communal and private living spaces. Amenities help residents stay comfortable and safe, and enrichment programs aim to offer engagement and purpose. The team is led by administrator Holly Self and centers its work on long-term and rehabilitative care, always trying to look after the unique needs and well-being of each resident.

Severe neglect, understaffing, COVID outbreak
I placed my grandmother here and it was a nightmare. Staff ignored COVID protocols, some worked while sick, and there was an outbreak - my grandmother caught COVID. The place is grossly understaffed: missed baths, skipped meals, meals/logging omissions, dirty diapers on the floor, bedsores, falls left on the floor, delayed/omitted X-rays and UTI tests that led to ER visits, and even reports of theft. Administration promises but never follows through; communication and supervision are poor. A few CNAs were caring, but overall the neglect and safety risks make me say: do not bring your loved ones here.
