    Caring staff but inconsistent leadership

I've been here nearly four years. The nursing and activities staff are warm, caring and attentive - meals, varied activities, nicer furniture/lighting and improved health outcomes (fewer UTIs, more smiles) - and rooms are spacious and generally clean. That said, administration and communication are inconsistent: staff can be rude or unhelpful, clothing has gone missing, hydration and sanitation lapses and odors occur, and there have been abrupt transfers/lock-in episodes; some issues get fixed quickly, others reappear. Overall I appreciate the care and engagement but would caution that leadership, communication and basic supplies/cleanliness need improvement.

    About Forest Hills Center

Forest Hills Center sits at 2841 E Dublin Granville Rd in Columbus, Ohio, and provides skilled nursing, rehabilitation care, assisted living, memory care, adult day services, and respite care, so there's a good mix of options for those who need different levels of support, and you'll find that Dr. Douglas Scharre serves as the medical director, he's a board-certified neurologist with a lot of experience in dementia, Alzheimer's disease, Parkinson's disease, and neurobehavioral disorders, which is helpful since a lot of the care here focuses on memory issues and related needs. The staff includes nurses who are on-site at all hours, day and night, offering medical support for different health situations, and there's a therapy team on-site too, offering physical, occupational, and speech therapy if someone is working to get stronger after being sick or hurt. The place is secure; everything's pretty much enclosed, and there are safety features made to keep folks from wandering off, which can be a concern for people living with dementia or Alzheimer's.

The building and its grounds are well-kept, with walking paths for residents to enjoy some fresh air safely, and the daily routine usually includes a full calendar with activities like exercise, arts and crafts, music, group outings, game nights, dancing, and live entertainment to keep everyone involved, no matter their abilities. Forest Hills Center also offers a range of healthcare services right there, such as podiatry, dental, optometry, audiology, wound care, medication support, and counseling, plus help with everyday things like getting dressed, eating, and taking medicine. Meals are prepared by a chef, so residents get hot food every day, and there's an on-site beauty and barber shop for haircuts and grooming, with transportation services if residents want to get out for appointments or outings.

For people with memory needs, there's a program that provides both long-term care and adult day services, and every resident has an care plan developed by staff who look at their physical, mental, and cognitive health to make sure everything fits their situation, and there are also staff trained especially to care for dementia. Forest Hills Center has certified professionals, and they all try to help residents reach whatever their potential is, whether that means more independence or just staying comfortable day to day, and they can take care of short stays, rehabilitation after the hospital, longer-term residential care, or just daytime support if families need that for their loved ones. The place has a map pin feature to help people find it, and the online provider directory gets updated about once a month. Some staff speak more than just English, although the details on available languages aren't clear, and right now the center's not taking new patients, but they keep the information up to date for everyone's reference.
