    Mixed care, highs and lows

    I've had mixed experiences: caring, friendly staff, a convenient location, active events and often clean rooms and decent food, but persistent problems too - rude or unprofessional employees, chronic understaffing, noisy nights, small/crowded rooms, inconsistent cleanliness, and worrying care/medication lapses. Because of that I can't confidently recommend this place without confirming current staffing, inspection records, and visiting in person.

    Siena Woods Care Center does a lot to care for seniors and has a wide range of options you don't always see under one roof, with services including Independent Living, Assisted Living, Memory Care with their Monarch Memory Care program, Home Care, Adult Day Services, Long-Term Care, Home Health Care certified by Medicare, and Hospice Care, and there's even help for people who need a place to stay just for a short recovery or respite. You'll find certified beds for 99 folks and on average about 85 residents each day, with private and semi-private rooms available, Medicaid Waiver studio apartments, and assisted living with studio and one-bedroom apartments depending on what a person might want or need. There's a big focus on safety and comfort with things like a sprinkler system, safety features, and secured memory care units, and everyone can use washers, dryers, Wi-Fi, or just sit down for a meal three times a day, with snacks too. Personal care's built in, so the staff helps with bathing, toileting, walking, wheeling, dressing, and grooming, plus there are laundry and cleaning services, and the team handles medication reminders and administration, wound care, podiatry, and even hospice for those who need it near the end of life.

    What's nice is that Siena Woods takes on complicated needs-you'll find programs for post-acute care after heart or lung problems or surgery, short-term rehabilitation between hospital and home, long-term skilled nursing with nurse coverage around the clock, and therapy services covering physical, speech, and occupational needs, along with outpatient help for folks who don't live in the building but still want treatment. Their Monarch Memory Care program for people living with Alzheimer's and similar problems uses practices from the Alzheimer's Association and centers care around each person's needs, while also working to keep everyone as active and connected in a safe setting as possible, and there's even a secured unit for memory care. The community provides lots of activities-arts and crafts, social programs, education, games, and a fitness center, plus there's a beauty salon and barber shop if you want that, and guests can even park when visiting, and flexible meal plans go along with a dining room.

    Siena Woods connects with St. Matthews Care Center, works as part of Exceptional Living Centers and under the managerial care of Walter Watts since February 2023 with Taylor Shaw serving as the executive director, and the facility links to Lexington Health Management as well. They do use a diverse staff and aim to keep relationships strong while looking after everyone's wellbeing, with a focus on respect and dignity, and you'll find personal care assistants on hand. There's always some room for improvement, as they've faced health inspection deficiencies, including four reports linked to infection control, pharmacy practices, and making sure food and drugs are handled right, and the last report was in April 2025, so that's worth knowing, and it's a good idea to ask about what they're doing to improve. Nurse turnover was at 59.1% and nursing hours per resident per day was 3.85, and they average about 85 residents a day, so it's a fairly busy place. They do accept a range of payment sources, including private funds, long-term care insurance, and veterans benefits, as well as Medicare and Medicaid, and have 24/7 admissions. Meal services, transportation, housekeeping, maintenance, and therapy are on offer for all residents' needs. The mission is about adding quality and compassion to each day, and while no place is perfect, Siena Woods Care Center covers a wide range and tries to give older adults choices, support, and dignity as they age.
